Description

JOB SUMMARY: Responsible for the daytoday support of all Interdisciplinary Team (IDT) functions and coordination/support of the IDT meetings and services. Collaborates with Quality Assurance Manager and Center Managers for tracking of service delivery and regulatory compliance.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  1. Assists Center Managers in the coordination of PACE program activities including assessments care planning and concurrences preparation for IDT conferences daily morning briefings service delivery requests and all related followup tracking and documentation.
  2. Assists in the preparation and facilitation of the IDT supporting it to achieve its goals and ensuring ongoing operational efficiency by preparing the daily agenda taking minutes and daily tracking of ER visits and Hospitalizations. Monitors IDT approved deadlines goals and timelines.
  3. Assists Quality Assurance Manager with Participant Incident Reporting process including obtaining and maintaining incident records tracking of Quality Assurance (QA) Incident process and following up with staff to ensure assessments and interventions are completed. Provide Fall Data information to IDT and Fall Review Committee.
  4. Assists Quality Assurance Manager with administrative support including data entry providing census reports scheduling meetings and taking minutes for QA committees (QIC MAC PAC Infection Control) facilitating mailers and other communications and any other reporting or data entry assigned.
  5. Assists Quality Assurance Manager with preparation and participation in Centers for Medicare & Medicaid Services (CMS) and Department of Health Care Services (DHCS) audits and develops corrective action plans.
  6. Supports Quality Assurance Manager in the process of receiving documenting and resolving medical and nonmedical grievances and appeals by participants and/or their representatives (e.g. reports of participant rights violations grievances appeals and other issues related to services and quality of care).
  7. Coordinates QA activities related to new enrollees as well as voluntary and involuntary disenrollment.
  8. Assists Quality Assurance Manager with tracking of staff required trainings and inservices including providing support with scheduling and conducting emergency evacuation drills.
  9. Demonstrates respect for and promotes participant rights including dignity selfdetermination access to care confidentiality and independence.
  10. Participates in the programs policy and procedure development.
  11. Acts in a manner consistent with agency values.

Qualifications

EDUCATION &/or EXPERIENCE:

  1. Bachelors degree in Healthcare Administration Quality Assurance or similar field preferred; may substitute equivalent experience.
  2. Minimum one (1) year working with a frail or elderly population preferred.

LICENSES OR CERTIFICATES:

  1. Current CPR and First Aid certification within six (6) months of hire.

PRESCREENING REQUIREMENTS:

  1. Requires clearance of a DOJ and FBI criminal history background check
  2. Preemployment physical and PPD skin test.
  3. COVID19 and Influenza (flu) vaccines preferred.

JOB SKILLS:

  1. Knowledge and experience working with the physical social and mental health programs operating within a licensed health facility or clinic.
  2. Skills knowledge and abilities related to the health mental cognitive and social needs of the participant population being served by the program.
  3. Ability to provide care for the frail and elderly and interest in working with elderly and disabled adults to enhance and encourage their independence.
  4. Ability to function effectively within a multiservice communitybased nonprofit agency.
  5. Ability to comply with program requirements and applicable government regulations.
  6. Sound verbal and written communication skills to convey information effectively. Ability to communicate effectively and openly with other team members on participants status and needs.
  7. Excellent interpersonal skills that result in maintaining a positive attitude team building and contribution to effective work relationships successful negotiation and conflict resolution with coworkers management participants and community contacts including referral sources.
  8. Ability to use good judgment resourcefulness flexibility and problemsolving skills.
  9. Ability to work independently and to lead a diverse team of various occupations. Strong administrative organizational and supervisory skills. Ability to train and delegate responsibilities among employees.
  10. Ability to prioritize and complete duties within an agreed upon time frame and adjust personal schedule as required and adapt appropriately to changes in priorities and workload.
  11. Ability to incorporate an understanding of detailed requirements in work activities. Ability to collect and analyze data.
  12. Ability to coordinate advocate and cooperate with other agencies for seniors with tact and diplomacy.
  13. Embraces diversity and has knowledge of cultural factors affecting care of diverse ethnic groups.
  14. Strong basic computer skills (Microsoft Outlook Word Power Point and Excel). Ability to navigate use and maintain database software programs.

PHYSICAL REQUIREMENTS:

  1. Ability to sit at a desk and interact with a computer screen for extended periods of time.
  2. Hand and arm strength sufficient to operate a keyboard for several hours each day.
  3. Physical abilities sufficient to move between different staff work areas communicate with coworkers and the public operate a computer produce reports talk on the telephone and travel to other agency worksites.
