Job Description

Core Schedule:MondayFriday 9am5:30pm could change based on needs of clients
Role Summary: Provides planning implementing monitoring and evaluating actions required to meet the young adults overall treatment needs. In addition s/he has the task of managing the Extended Reach system related to the filing process of new and existing clientand is responsible for the care of the efile throughout the clients stay within the supervised independent living program.

Essential Functions:

Responsible for coordinating and connecting care for all clients within the Supervised Independent Living Program.
  • Assessing the young adults needs ensuring appropriate connections to treatment team members and outside resources are initiated and followed up on. Ensuring outside treatment team members are contacted within 3 days of youths admission to ensure strong connections are made with all treatment team members.
  • Serves as the initial point of contact for a variety of referral sources including DCBS workers and community partners.
  • Facilitates weekly treatment team meetings coordinates treatment team schedule and invites to team members.
  • Communicate and facilitate with appropriate treatment team members the young adults needs treatment concerns and safety issues as outlined in BGH Policy and Procedures. (i.e bed holds clothing needs medical hospitalization CSA services OT services Vocational Rehab)
  • Open and maintain case files for youth that include all documents required. Make sure young adults understand program requirements goals and treatment plans as well as grievance forms clients rights release of information and other relevant forms.
  • Responsible for updating and maintaining DCBS worker and client contact information and sending internally
  • Be apart of new client admission with Program Supervisor and other leadership.
  • Ensure clients complete necessary paperwork. Note the clients progress performance and achievements so this information can be used when compiling case notes case review/audit documentation progress reports and other critical documentation.
  • Responsible for monthly and weekly connections to DCBS Aetna and other possible outside supports
  • Completes monthly state required documentation to include 1294 As and Bs.
  • Collaborate and implement discharge plans with treatment team from the beginning of admission. Exploring and connecting additional outside supports for young adult to be successful after discharge.
  • Responsible for participating in URC biweekly meetings and collaboration efforts within BGH programs
  • Assist with transition from one program within BGH to another via appropriate documentation and communication.
  • Completing documentation at discharge to ensure representation of all needs are noted for a successful transition for young adult.
  • Sends discharge notifications to program supervisor or their designees.
  • Meets regularly with the immediate supervisor as a means of enhancing professional growth reviewing and processing the provision of care coordination services and dealing with appropriate administrative issues.
  • Coordinate referrals for Community Support Associate (CSA) and Occupational Therapy (OT) services and ensure all documentation required for billing is accurate timely and submitted for reimbursement in a timely manner.
  • Participate in OnCall Rotation

Qualifications:

    1. Education A Bachelors degree in social services or other related/relevant field. Masters Degree preferred.
    2. Experience/Credentials Minimum of two years experience working with older adolescents and young adults in a residential treatment mental health/psychiatric hospital or preindependent/independent living setting. Excellent verbal and written communication and organizational skills including proficiency with appropriate computer software/hardware (i.e. Windows Microsoft Excel Outlook Power Point Word and other relevant software/hardware).
    3. Other Interact professionally with BGH employees clients visitors government agencies and the general public. Maintain confidentiality of all client information. Must be at least 21 years of age and legally able to work in U.S. Valid drivers license and insurability by BGHs insurance carrier required. Successful completion of all background screenings and preemployment drug screen. Successful negative postcontingent offer and annual TB screenings.
Physical Demands: While performing the duties of this job the employee is required to: stand walk and sit; use hands to manually manipulate handle or feel objects tools or controls; reach with hands and arms; climb stairs; balance; stoop kneel crouch or crawl; talk and hear; taste or smell. The employee must occasionally lift and/or move up to 40 pounds. Specific vision abilities required by the job include close vision distance vision color vision peripheral vision depth perception and the ability to adjust focus.
Ability to read write and converse in English.

Work Environment: While performing the duties of this job the employee is exposed to clients apartments communal areas and a standard office environment. Clients living spaces may range from impeccably clean to less than appealing regarding order and cleanliness. The noise level in the work environment is usually moderate. The employee will come into contact with BGH clients staff members Board members volunteers and other members of the community.
