Job Description

Weekdays 1st Shift!

Job Summary:

Performs daily case management for youth participating in the Massachusetts Youth Diversion Program (MYDP). Acts as main point of contact for youth in diversion families service providers and diversion referrers.

Essential Job Functions:

  • Adheres to all applicable state and federal regulations; ensures compliance with the organizations policies and procedures code of conduct and all applicable regulatory requirements.
  • Meets with youth referred for MYDP and families to explain the process and expectations.
  • Completes risk/needs assessments and gathers information from collaterals to effectively craft an individualized diversion agreement.
  • Matches youth to appropriate intervention for case plans.
  • Monitors youth progress throughout the diversion process.
  • Meets with youth at a predetermined frequency in the community or other approved locations to support youth engaging in interventions identified on their case plan.
  • Keeps in contact with youth at a predetermined frequency and communicates with youth about expectations at each stage of diversion.
  • Communicates with other system stakeholders and service providers in the community.
  • Builds community connections and establishes partnerships for future programming.
  • Attends court hearings and other meetings relevant to youth needs.
  • Distributes collects and inputs data on diversion cases for purposes of evaluations.

Other Duties:

  • Performs additional duties as directed by management.
  • Attends or completes all required meetings and trainings.
  • Participates in regularly scheduled individual and group supervision.

Qualifications Experience and Education:

  • Bachelors degree in criminal justice social work psychology sociology or related field.
  • Minimum one year of experience working with atrisk youth.
  • Excellent knowledge of community resources.
  • Basic knowledge of/experience in restorative justice practices.
  • Basic knowledge of/experience in screening/assessment tools case planning and evidencebased practices.
  • Strong organizational and time management skills.
  • Valid Drivers License and use of personally insured vehicle for agency business.
