What will your responsibilities be
In this role you will work within the Discharge Assessment and Review Team (DART) at Doncaster Royal Infirmary supporting discharges from hospital and the emergency department for adults aged 18 and above. You will carry out strengthbased assessments under the Care Act 2014 and Mental Capacity Act 2005 focusing on safe discharge planning with a Home First ethos. Collaborating with health professionals you will identify and arrange appropriate support whether returning home accessing rehabilitation or further assessment in a bedbased setting. You will operate across various settings to facilitate effective discharge planning ensuring timely personcentred outcomes aligned with hospital discharge policy and best practice.
