We are seeking an experienced MSW Case Manager to support its community-based PACE (Program of All-Inclusive Care for the Elderly) clinic. The case manager will serve as a vital member of the interdisciplinary team (IDT) coordinating care supporting transitions and ensuring safe discharge planning. This role is ideal for licensed social workers with strong skills in assessments discharge planning and care coordination for elderly and vulnerable populations.
Responsibilities:Conduct initial and 180-day biopsychosocial assessments of participants in the PACE program
Coordinate hospital and SNF discharges in collaboration with discharge planners and the IDT
Develop individualized care plans and goals in alignment with IDT recommendations
Monitor participant progress and ensure continuity of care
Provide psychosocial support counseling and education to patients and caregivers
Facilitate referrals to community-based programs and long-term care facilities
Advocate for participants by navigating Medicare Medicaid and social service systems
Document care planning and service delivery accurately in EPIC
Ensure culturally competent care for patients with complex medical psychiatric or socio-economic needs
Minimum 2 years of relevant experience required
Travel experience required first-time travelers not accepted
Must be licensed in Washington State as LICSW LSWAIC or LASW only one is required
MSW degree required
BLS Certification required
Epic charting experience required
Community hospital or outpatient clinic experience required
Must be able to coordinate across multiple care settings and with external providers
RTO must be included at time of submission
No weekends on-call or holidays required
Masters degree in Social Work (MSW) from an accredited program
2 years of case management experience in community outpatient or hospital settings
Experience with vulnerable populations discharge planning and multidisciplinary coordination
Must hold one of the following WA State licenses:
LICSW (Licensed Independent Clinical Social Worker)
LSWAIC (Licensed Social Worker Associate Independent Clinical)
LASW (Licensed Advanced Social Worker)
BLS Certification required
Case Management & Utilization Review:
Care coordination discharge planning continued stay reviews prior authorizations
Familiarity with admission criteria appeals denials and benefits eligibility
Assessments & Psychosocial Evaluations:
Biopsychosocial assessments geriatric depression scale dementia screening
Development of problem lists and goals
Protective Services & Risk Management:
Knowledge of APS guardianship POA emergency detention
Care Coordination & Interdisciplinary Communication:
Referral to rehab LTC and social/community agencies
Participation in care conferences and documentation in EPIC
Special Populations:
Dementia psychiatric disorders drug/alcohol dependency
Cultural sensitivity vulnerable adult safety interventions
Community Resource Navigation:
Medicare/Medicaid HCBS Social Security aging agency programs
Counseling & Crisis Intervention:
Short-term therapy patient education discharge contingency planning
