We are seeking an experienced MSW Case Manager to support a community-based PACE (Program of All-Inclusive Care for the Elderly) clinic. The ideal candidate will manage a caseload of medically complex patients facilitate care coordination across settings and serve as a key member of the interdisciplinary team (IDT). This position requires Washington licensure (LICSW LSWAIC or LASW) and prior travel experience.
Responsibilities:Conduct comprehensive psychosocial assessments at intake and 180-day intervals
Collaborate with hospital discharge planners to develop and implement safe discharge plans
Coordinate SNF discharges and transitions of care for rehab or skilled needs
Develop and update individualized plans of care in collaboration with the IDT
Provide education and counseling to patients and families on community resources advance directives and post-acute care options
Refer patients to long-term care home/community-based services and rehab facilities as needed
Document all activities communications and assessments in EPIC
Participate in IDT meetings to ensure continuity and quality of care
Address psychosocial barriers and promote patient well-being and independence
Must hold one of the following Washington State social work licenses:
LICSW Licensed Independent Clinical Social Worker
LSWAIC Licensed Social Worker Associate Independent Clinical
LASW Licensed Advanced Social Worker
MSW degree from an accredited program required
BLS certification required
2 years of relevant case management experience required
Travel experience required first-time travelers will not be considered
Must be familiar with discharge planning care coordination and outpatient case management
EPIC experience required
Community hospital experience required
LTAC and Trauma Level I experience preferred
Patient ratio: approximately 1:50
No weekends no call no holidays
Masters Degree in Social Work (MSW)
Minimum of 2 years in community-based case management or hospital social work
Experience working with older adults vulnerable populations and interdisciplinary teams
Must have one active WA State license at submission: LICSW LSWAIC or LASW
BLS Certification required
