DescriptionThe Quality Coordinator under the general direction of the Manager Quality develops and implements quality improvement programs for PACE Baton Rouge including annual review and update of quality improvement plan development and implementation of program wide and center specific quality indicators data management education and training of program staff around quality improvement preparation of routine and special reports and coordination of improvement efforts arising from monitoring activities.
Responsibilities- Quality Assessment and Performance Improvement
- Oversees and coordinates the development and implementation of program wide and center specific quality indicators.
- Responsible for data management for Quality Improvement activities including coordination of collection entry and analysis and assessment of data.
- Coordinates with National initiatives in PACE Quality management including developing of corrective action plans for CMS reports
- Enters data needed on a quarterly basis into HPMS required by CMS.
- Coordinates the necessary entries for the HOS survey on a yearly basis and works with all agencies necessary to improve the outcomes of PACE.
- Assists in interface between consumers and program management for quality related grievances.
- Education
- Ensures the Quality and Infection Control Committee meets monthly and is chairman of both committees providing data education and corrective action plans to the committees.
- Ensures the Contract entities that provide Participant Direct Care are oriented to PACE and have education given to them at set intervals and as needed when situations arise.
- Provides ongoing education to staff as related to Quality Assurance Performance Improvement
Qualifications- 2 years in healthcare quality improvement.
- BS in health related field required.
