Internal Rate G8: $55201.79Application Deadline:May19 2025 by 11:59 pmProgram Overview First Step to Home Housing & Homelessness ServicesFirst Step to Home is a transitional program with housing as an enabler for men 55 years and up who have been street involved and experiencing mental health and/or addictions issues. The programprovides onsite psychosocial primary care supports and services including: intensive case management; harm reduction and addiction supports; therapeutic and supportive counselling; 24hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.
Job Overview
This position offers services to tenants of Supportive Housing Programs living in their own bachelor style unit. All participants have prolonged mental health and/or addictions; have experienced homelessness and who are working to maintain affordable and sustainable housing. This position is expected to carry out the services in accordance to WoodGreens policies and procedures procedures Ministry of Health and health related authorities and all relevant legislation. This philosophy of this work is in keeping with WoodGreens mission to deliver services that promote wellness and selfsufficiency reduce poverty and inequality and build sustainable communities.

This position will be responsible for working as part of a team of Client Support Workers Case Managers and Clinical Support work who provide case management services and crisis intervention support.

What You Will Do
  • Undertake needs assessments and develop case plans with individuals in the program.
  • Provide comprehensive case management services; assessment personal goal setting short and longterm planning informal counselling instrumental/practical assistance followup supports crisis intervention care/service coordination case conferencing and referrals.
  • Assist tenants with mental health issues by providing a variety of daytoday physical emotional and social supports life skills assistance information and resources.
  • Identify and address mental health issues connecting them with primary care and other needed resources and support them to utilize informal and formal support networks appropriately.
  • Work closely with staff and clients on strategies that support clients with substance use and mental health issues to maintain their housing and advocate on behalf of clients when required.
  • When required accompany clients to offsite medical appointments and any other communitybased services and provide emotional and logistical support to clients during offsite visits ensuring they understand appointment details advocate for their needs and stay engaged throughout the process
  • Link clients to various community resources and facilitate appropriate referrals to other community support services when required.
  • Ensure the coordination of services and a continuum of care.
  • Liaise consult and collaborate with other program area staff and service providers.
  • Monitor the housing challenges and ability of each resident to adhere to the policies and protocols and develop a team strategy for intervention when problems occur in accordance with housing first and eviction prevention principles.
  • Complete statistics/reports and maintain all records regarding program/activity enrollment.
  • Ensure that the antiracism objectives and community development principles of the organization are achieved in the program.
  • Gather statistical and narrative information required for the Service and organization. Participate in ongoing training and education.
  • Participate in regular supervisory sessions and annual performance appraisals.
  • Might be exposed to secondhand smoke behavioral & cognitive outbursts death dying drug use pestrelated concerns etc.
  • Carry out relevant tasks as directed by Supervisor Manager and Director.
What You Bring to the Team
  • Bachelors degree in Social Work or a related discipline.
  • 3 years related experience in mental health and harm reduction housing help and working with those who have experienced homelessness.
  • Proven ability in case management informal counselling and crisis intervention.
  • Strong housing help skills and ability to support individuals to incorporate a harm reduction strategy in obtaining and maintaining their housing.
  • Proven ability to minimize barriers through service delivery for low income individuals with mental health and substance use issues

  • A thorough understanding of clinical practices and care models.
  • Applied computer skills (MS word excel and Internet case management software Pirouette)
  • Ability to work weekends statutory holidays on an exceptional basis where and if applicable
  • Might work in environments where there are pests whereclients use substanceswherein clients have incontinence issues.
  • Clients may exhibit responsive behaviours including profanities and expletives.
  • Able to lift 40lbs.
