Salary: 27000 grossper annum plus 7% employers pension contribution

Hours of work: 36.5 hours hours per week

Community First is a registered charity (288117) based in Devizes. As a member of Action with Communities in Rural England we support communities across Wiltshire and Swindon. Our Patron is Her Majesty The Queen.

All about the role:

The role focuses on providing practical and emotional support for Young Carers in Wiltshire aged 519 years. The post holder will be required to work with young carers understand their individual needs and develop a positive trustbased relationship with them and support them throughout their journey with our service.

You will have experience of successfully delivering one to one and group work support for young people. Be creative adaptable with strong interpersonal skills and able to work effectively both independently and as part of a team.

The role will include regular evening weekend and school/college holiday working.

Main Responsibilities

  • Work with young carers referred to the service to cocreate and regularly review individual support plans.
  • Deliver a range of 1:1 support including coaching and mentoring programmes and time to talk sessions which engage and support young carers to overcome the challenges they face.
  • Organise and deliver a range of respite activities including webinars clubs day multiday & residential experiences held during evenings weekends and each school/college holiday period. which engage motivate and provide varied personal development opportunities for young carers.
  • Manage groups of young carers and be responsible for their care while participating in the service offer including residential provision.
  • Maintain a comprehensive knowledge of external provision in order to provide information advice and guidance transition support signposting and access to other services and agencies when appropriate for young carers and families.
  • Foster a young personcentred approach at all times ensuring there is no discrimination or bullying.
  • Identify and engage with Wiltshire primary schools to promote and enable the successful roll out of theLocal Young Carers Friendly Award & National Young Carers in Schools award.
  • Provide relevant resources and support to enable Wiltshire primary schools to achieve the Local Young Carers Friendly Award & or National Young Carers in Schools award.
  • Meet all administrative responsibilities within the post holders area of work including ensuring case work and MIS data is recorded accurately and in a timely fashion (currently Views and Cascade).
  • Manage a case load of young carers and advocate on behalf of their needs.
  • Support the delivery of accredited non formal learning programmes for young carers including where appropriate leading workshops activities and sessions.
  • Support the development of our young peoples voice groups and the ongoing development of Peer Mentors Young Listeners and Young Leaders.
  • Provide statistical information and input into reports and impact evaluations
  • Support Young Adult Carers aged 16 years to complete transition planning and work in partnership with the Carer Together Wiltshire team to support Young Adult Carers.
  • Maintain current provider relationships and develop strong partnerships to generate new referrals funding and new activity
  • Ensure that all activities are risk assessed and approved using the appropriate forms and process and ensure the safety of young people whilst participating in activities.
  • Organise and provide as needed transport equipment clothing and healthy meals for young carers participating in service activities.
  • Involve young carers families and delivery partners in formative evaluation and support the preparation of funding applications to Trusts statutory organisations and national funding streams.
