This position is based at Law Harrington Senior Livingbut you will be expected to attend meetings and provide some services at 401 Branard St.
A. Responsiblities
- Verify client eligibility for program services and ensure compliance with senior services funding sources. Compile or delegate and supervise funding source reports.
- Coordinate SPRY social educational recreational activities and programming at Law Harrington main campus and in the field. Coordinate congregated meals through the Diner Manager. Coordinate food distribution programs including Houston Food Bank Senior Boxes and supplying Law Harrington emergency pantry. Lead tours of Law Harrington to external vendors community leaders and partners as necessary.
- Oversee volunteers and student interns at Law Harrington Senior Living and for the senior programming (meals activities community service etc.)
- Client satisfaction survey collection and outcomes results to the Chief Programs Officer.
- Cultivate relations with other senior provider and community entities and conduct LGBT senior cultural competency training.
- Develop with the Management Team Chief Programs Officer or CEO clinical and administrative policies and procedures within any standards of care that guide the program services and establish accountability to the Management Team and Board of Directors for the services. Define the role of each discipline within the program as applicable. Implement approved clinical and administrative policies and procedures within the program. Ensure that client rights are honored.
- Develop/write the technical portion of any grant or proposal to fund the program by the due date assigned. Complete program reporting for any grants by the due date.
- Participate with the Finance Director Development Team and/or CPO in selection of any vendors collaborators or referral sources for services not provided by the program.
- Work with the Communications and Marketing Director to market the program in the community and among professionals and organizations through attending professional and service provider meetings mailings and/or speaking engagements. Represent the Center with other service providers and networks in instances when issues that fall under the auspices of the program are involved and as approved by the CPO.
- Integrate all above programs into the overall functions of the Center. Coordinate the programs with other programs within and outside the Center.
- Program review and improvement activities include in all program components assigned to the Senior Program: planning and reviewing the process at least quarterly setting improvement priorities systematically assessing program performance implementing improvement activities based on the assessment and maintaining achieved improvements. Oversee the quality management of the program through continuous assessment and quality improvement by outcomes measures review supervision and random file review. Review and act upon any reports about the program presented by funding sources licensing body Quality Improvement or review or advisory committees. Communicate program and performance improvement recommendations to staff during monthly Program Meetings.
- Manage staff for TransWellness program. Guide TransWellness Program Manager in facilitating events budgeting hiring and other programmatic and managerial responsibilities. Attend external / networking meetings as required.
- Facilitate programming for United Way ICJ program. Manage Barrier Buster fund ICJ tool client assignment and other United Way required programming and technologies. Manage team of Navigators.
- Serve as liaison with US State Department / IREX for CEE and CSP international exchange fellowships. Interview and place prospective Fellows. Supervise Fellows and guide programmatic presentation and project development during their 3 months. Serve as agency representative with IREX and other external stakeholders. Assign Peer Collaborator and facilitate interactions with Fellows. Assist Fellows with navigating Houston and becoming connected within the Montrose Center.
- Assist client and caretaker in the implementation of the plan brokering services where needed and presenting the case to service provider group for consultation with other involved agencies as appropriate.
- Serve as liaison between Law Harrington Senior Living and the Third Ward community. Attend community meetings and assist in facilitating collaborative programs as necessary.
- Enter all encounters the day of the session and complete progress notes within 72 hours of the session. Keep timely and comprehensive progress notes. Conduct discharge planning with the participation of the client and complete a final discharge plan and discharge summary and give a final outcomes measurement at the time of termination of services
- Determine with the Management Team the qualifications for program staff. Interview and recommend to the COO qualified and competent new staff for vacant positions within the programs including SPRY Navigation and TransWellness program. Orient all new employees and students to the program guidelines philosophy and requirements. Determine the competence at hire and at review of program staff. Supervise the SPRY Outreach Worker and activities Navigators and TransWellness Manager. Will act as the main contact with the LHSL property management.
- Supervise program staff within the program and agency by evaluating performance as outlined in the Personnel Policies and job descriptions by providing guidance and support for cases as required by licensing but no less than on a biweekly basis. Communicate interpret and enforce program and administrative policies with supervisees at orientation and as new policies are developed. Communicate any staff concerns regarding policies and procedures to the Management Team in the weekly meeting.
- Orientate new counseling staff to the program rules standards of care and philosophy. Participate in developing inservice presentations and approving continuing education for program employees.
B. Education Training SkillsKnowledge and
Abilities
- Graduation from an accredited college or university with a Bachelors degree required in Social Work Psychology Social Services or equivalent (3) years experience in senior services.
- Encouraged to become a member of a generally recognized professional association or discipline.
- Detailed knowledge of community resources; special treatment issues for the lesbian gaybisexual and transgender community; issues relating to seniors.
- Access to reliable transportation a current Texas Drivers License and auto insurance. Ability to mentally and physically cope with stresses caused by regular exposure to the effects of clients behavioral health issues and respond appropriately to client crises and seek help in coping when needed through supervision case consultation or counseling when needed.
