Job Description

The HWD (LVN or RN) oversees and supervises the care of all the residents. The HWD also provides direct resident/patient care. The Health and Wellness Director oversees the health services department and ensures resident care needs are met by coordinating resident care managing health services systems and providing supervision to health services team members. Promotes a positive physical and psychosocial environment for the residents

1. Creates a residentcentered approach to the planning and delivery of services to residents supporting residents rights to participate in decisionmaking and treating residents with dignity and respect.

2. Responsible for accurate completion and use of level of care determination tools to ensure residents receive the care and services they require and desire.

3. Performs assessments on health functional and psychosocial status of residents. Participates in the development of individualized service plans. Proactively manages care and services for each resident maximizing the residents opportunity to remain in the assisted living environment. Problemsolves as part of the management team to case manage and meet the needs of residents.

4. Assesses or evaluates residents with reported changes of condition. Provides team member direction service planning and coordination of care to meet the residents changing needs.

5. Collaborates with the management team on the move in of prospective residents taking into consideration the needs of the prospective resident current community acuity and staffing levels. Provides inperson premove in assessments if necessary.

6. Ensures that resident care needs are met by coordinating services with other professionals including physicians discharge planners nurses home health agency staff case managers hospice and state agency personnel. Oversees the arrangement of ancillary services such as pharmacy podiatry lab and xray as needed.

7. Communicates regularly with residents families regarding health care needs. Provides education and support to residents and families.

8. Provides supervision and training to health services team members including personal care and medication management. Manages the delegation of nursing tasks as allowed under applicable state nursing laws and practice acts.

9. Partners in the investigation and root cause analysis of incidents. Collaborates with the management team on the development and implementation of appropriate interventions.

10. Oversees the documentation and communication of resident care and services. Ensures resident records are completed in a timely manner and maintained according to company policies and state regulations.

11. Assists in the management of infection control. Provides training to staff on infection control practices investigates and reports incidents of infection and tracks infection data.

12. Manages and oversees health services systems. Maintains uptodate knowledge of all rules regulations and policies pertaining to resident care. Stays informed of industry and health care trends and standards applying new knowledge to meet the changing needs of residents.

13. Provides oncall support and phone triage for team members questions regarding resident care.

14. Participates in leadership team by actively contributing to decision making incident follow up community and companywide initiatives and attending leadership team meetings such as daily stand up clinical meeting Quality Assurance Performance Improvement (QAPI) and other meetings as required.

15. Acts as an ambassador by representing the community in a positive manner to outside agencies and participating in community events and social functions.

16. Works within scope of license with RN oversight as needed.

17. Manages the schedule and timekeeping for care staff as well as personnel records.

18. Performs other duties as assigned.

The Health and Wellness Director is responsible for supervising and managing the entire Health Services staff either directly or indirectly.
