Youll work remotely to guide clients through care options help coordinate services and ensure they receive the support they need.
This is a peoplefirst role ideal for someone who is empathetic detailoriented and experienced in clientfacing or case management positions.
Key Responsibilities:
Serve as the main point of contact for clients seeking care or support
Help clients understand and access available resources (medical behavioral financial etc.
Coordinate appointments referrals followups and service navigation
Maintain accurate documentation of client interactions and status updates
Communicate with care providers case managers and support teams
Follow up with clients to ensure needs are met and barriers are addressed
What Were Looking For:
U.S. citizenship required
2 years of experience in care coordination case management social services or customer support
Strong interpersonal and communication skills both written and verbal
Empathetic solutionfocused and able to work with clients from diverse backgrounds
Organized and techsavvy; comfortable using systems like Salesforce EHRs or CRM tools
Ability to work independently in a remote environment and manage multiple clients.
