Palliative Care Lynchburg
Introduction to the Role and Practice
We are seeking an exceptional and compassionate Nurse Practitioner or Physician Assistant to join our dedicated Palliative Care team. This is a meaningful opportunity to make a lasting difference in the lives of patients and families during some of their most vulnerable moments.
In this role you will provide holistic and supportive care for patients with advanced illnesses primarily in oncology neurology (stroke dementia etc.) pulmonary (COPD respiratory failure etc.) and gastrointestinal diseases. You ll be part of a collaborative interdisciplinary team focused on improving quality of life and managing symptoms.
You Can Expect
Weekends Only
5 8 patients per day a mix of new admits and follow up patients
2 Physicians and 4 APPs
Collaborate with families patients social workers and care teams across the system
A robust 3-6 month orientation process
Required Qualifications
Experience with disease process
Strong communication skills
Experience with symptom management
End of life care
Advanced care planning
