Develop implement and manage a systematic Corrective Action Program (CAP) that ensures effective closure of findings from incidents audits and observations. Conduct effectiveness reviews to validate that corrective actions are implemented and achieving intended risk mitigation. Track trend and report on corrective actions to identify systemic issues and drive continuous improvement.
Incident Investigation Program
Design implement and manage a formal Incident Investigation Program to ensure thorough root cause analyses and consistent incident reviews. Conduct effectiveness reviews of investigations to confirm root causes are accurately identified and corrective actions address underlying risks. Lead or support serious incident investigations (including fatalities lifealtering injuries and major property damage) with structured analysis methodologies. Train operational teams in investigation techniques such as TapRoot 5Why and Cause Mapping. Approve and oversee investigation reports and ensure communication and integration of lessons learned across all field operations.
Audit and Observation Program
Develop and lead a robust Audit and Observation Program focused on field activities critical equipment and highrisk operations. Analyze audit and observation data to identify trends systemic issues and improvement opportunities. Collaborate with operations leadership to build and execute corrective action plans based on audit findings.
Crew Competency and Risk Modeling
Develop competency frameworks that align with operational risk profiles and critical task performance. Partner with operations and training teams to assess and verify crew readiness. Design and maintain risk models that assess crew and operational risk exposures based on experience qualifications and operational factors.
Readiness Dashboards for Field Operations
Develop and maintain realtime readiness dashboards providing visibility into crew competency outstanding corrective actions and leading safety indicators. Ensure dashboards are integrated into daily operational management and decisionmaking processes.
