Job Description

Use your passion for mental health and play a key role in the Psychosocial Support Program as a service navigator across the Nepean and Blue Mountains region!

Flourish Australia has an exciting opportunity for a Support Coordinator to join our established team!

The position available is:

  • Parttime(45.6hours per fortnight)
  • Working between Monday and Friday
  • Based in Penrith with occasional travel required between Windsor and Katoomba
  • Level 4on the Flourish Australia Enterprise Agreement 2018 equivalent to $85000 per annum (prorata) 11.5 Super TaxFree Salary Packaging

About the Opportunity

Support Coordination aims to support people with a permanent and significant disability that affects their ability to take part in everyday a Support Coordinator youwill establisha positive collaborative relationship with the person andtheir support network. You will assist the person to identify link with and coordinate supports in their localcommunities to build skills overcome barriers and achieve goals.

In this roleyou will give people more choice and control over how when and where those supports are provided. You will be fundamental inthe implementation of the persons plans and the achievement of their this position you will also berequired to ensure that people who have expressed an interest in accessing the NDISand/or receiving support from Flourish Australia are engaged and supported to access suitable services.

As the Support Coordinator for the Psychosocial Support Program your duties will include (but are not limited to):

  • Building connections with other local services and creating referral pathways
  • Assessing for suicide risk and having suicide prevention knowledge and training
  • Completing needs assessments both over the phone and inperson
  • Determining eligibility for people accessing the service for programs and services across the region
  • Being a part of different interagency meetings and inservices to promote Psychosocial Supports program and other Flourish Australia programs
  • Creating service directories for the Penrith Hawkesbury Blue Mountains and Lithgow LGAs
  • Connecting people to a range of different agencies and services in the region including NSW Health Government agencies community and other disability support services
  • Supporting with NDIS access requests and assisting in gathering supporting documentation for access
  • Following up all inquiries in a quick and professionally manner

Please note Successful candidates must maintain a Class C drivers license and proof of comprehensive insurance must be provided when required. You may be directed to use your private motor vehicle when anorganisational pool motorvehicle is will receive the Travelling Transport and Fares Allowance when using your private vehicle for work related purposes.

Selection Criteria

Essential:

  1. A tertiary qualification in the behavioural sciences or a healthrelated discipline.
  2. Demonstrated time management skills including prioritisation of competing tasks and theability to work innovatively to solve complex issues.
  3. Demonstrated understanding of the challenges faced by people with a lived experience of a mental health issue and how providing a recovery oriented service is of benefit to people.
  4. Understanding of the public sector and community based service systems relevant to people with a lived experience of a mental health issue and psychosocial disability and demonstrated experience navigating such systems and using influence to achieve positive results.
  5. Demonstrated experience in the provision of support and coordination for people with a lived experience of a mental health issue.
  6. Demonstrated strong communication advocacy and interpersonal skills including the ability to liaise effectivelywith a wide range of stakeholders facilitate outcomes and prepare correspondence & reports in clear and concise language.
  7. Current Australian Drivers Licence.
