Job Details

PRINCIPALES FONCTIONS ET RESPONSABILITS :

  • Fournir un soutien mdical aux patients et aux fournisseurs de soins de sant y compris mais sans sy limiter lducation sur des maladies et traitements spcifiques et la gestion des maladies;

  • Fournir un soutien tlphonique (incluant les appels de maintenance et dappui lobservance entrants et sortants) selon les besoins;

  • Gestion de la base de donnes et vrification continue des dossiers et de linformation saisie;

  • Administrer et adhrer des protocoles de gestion des cas incluant la surveillance clinique la collecte de donnes le suivicontinudes patients la conformitle suivi delinterruption ou labandon de traitement les rapports sur les effets indsirables et la liaison avec les fournisseurs de soins de sant primaires;

  • Se prparer et assister des sances de formation spcialise plus pousses sur des thrapies spcifiques donnes par lquipe pharmaceutique;

  • Travailler directement avec le spcialiste du remboursement et le mdecin pour obtenir des fonds pour les traitements de leurs patients; aider trouver dautres sources de financement au besoin;

  • Assister aux runions de ventes pharmaceutiques au besoin;

  • Assister aux runions de patients et aux confrences au besoin pour duquer les patients et les prestataires concernant les informations sur les maladies/les produits etles servicesde gestion de cas;

  • Travailler avec les associations de patients et les groupes de dfense;

  • tablir et maintenir des partenariats avec lquipe pharmaceutique;

  • Assurer la liaison avec les fournisseurs de soins de sant les pharmacies et les cliniques de perfusion et dautres organismes au besoin afin de coordonner les perfusions ladministration de mdicaments et rsoudre les problmes qui surviennent;

  • Entretenir des communications claires avec le gestionnaire lquipe et le client concernant la gestion des cas de patients;

  • Avec les gestionnaires participer au processus de formation de nouvelles infirmires gestionnaires de cas y compris mais sans sy limiter lobservation et la surveillance des appels;

  • Soutenir et collaborer avec le personnel dautres programmes en vue de llaboration et de la mise en uvre de programmes de gestion des maladies incluant ltablissement des exigences cliniques la rdaction de textes de manuels ou de dpliants pour les patients la dtermination des paramtres du programme de conformit des valuations des besoins et des protocoles;

  • Fournir des commentaires au sujet des protocoles bases de donnes processus et relations entre dpartements actuels

  • tre disponible sur appel en dehors des heures rgulires et le weekend 24/7 pour des consultations tlphoniques priodiques et des courriels en fonction des besoins du programme;

  • Se charger de signaler les effets indsirables aux units/dpartements qui lui sont confies;

  • Dautres fonctions et tches peuvent tre confies linfirmire gestionnaire de cas de temps autre selon les besoins;

  • Peut tre appel travailler sur appel ou de garde.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Provide support to patients and health care providers by offering information on the Patient Support services specific diseases and therapies based on manufacturer provided materials and product monograph;

  • Assist patients and caregivers in conducting assessments related to Patient Support services;

  • Act as a liaison between patients families and health care providers related to medication access.

  • Address general inquiries regarding program services disease states or product related queries based on provided materials;

  • Offer continuous support on disease states or productrelated questions to ensure patient adherence;

  • Refer the patient to their healthcare providers for clinical questions beyond program materials;

  • Document patient information and interactions within the electronic patient record;

  • Conduct ongoing file review and updates to reflect patient journey;

  • Follow casemanagement protocols including data collection patient followup therapy disruption/discontinuation monitoring adverse event reporting and liaison with primary healthcare providers;

  • Collaborate with the reimbursement specialist and the physician to secure funding for their patients therapies and explore alternative funding options;

  • Ensure patient services are coordinated and tracked according to KPIs resulting in expedited access to therapy;

  • Participate in specialized training sessions for specific therapies provided by the pharmaceutical team;

  • Attend pharmaceutical meetings patient meetings conferences and work with patient associations and advocacy groups as needed ;

  • Coordinate with health care providers pharmacies and infusion/injection clinics for treatment logistics and issue resolution;

  • Maintain clear communication with the manager team and the client regarding the management of patient cases;

  • Assist in the training process of new Nurse Case Managers and provide feedback on protocols databases processes and interdepartmental processes;

  • Report Adverse Events / Severe Adverse Events following program procedures;

  • The Nurse Case Manager will also be assigned other duties and tasks as required from time to time. May be required to be on standby/ on call as part of this role.

EXPRIENCE ET NIVEAU DDUCATION REQUIS :

  • Baccalaurat oudiplme dinfirmier(re) autoris(e) (IA) diplme dtudes suprieures ou exprience spcifique de gestion dun cabinet est prfrable avec cinq 5 annes dexprience en soins infirmiers;

  • Au moins deux 2 ans dexprience du contexte du remboursement par destiers publics ou privs ou dans les ventes (non requis mais un atout) la gestion des soins ou le soutien clinique dans lindustrie pharmaceutique ou exprience connexe;

  • Le(la) candidat(e) prfr(e) est un(e) infirmier(re) avec une exprience dans les soins de sant et la gestion de cas dans un cabinet mdical ou un hpital ou dans lindustrie de lassurance ou de la biopharmaceutique;

  • Bilinguisme (franais et anglais) est requis

  • Exprience en hmatologie et/ou nphrologie constitue un atout.

EXPERIENCE AND EDUCATIONAL REQUIREMENTS:

  • A Diploma in health care or a related field with a preference for Bachelors degree

  • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required.

  • Minimum of two 2 to five 5 years in the healthcare pharmaceutical insurance or related fields;

  • Ideal candidates should be nurses with healthcare and case management background in medical/hospital settings insurance or biopharmaceutical industry;

  • Bilingualism (French and English) is required.
