Job Description
Location: Santa Fe NM 87505
Weekly Pay Rate: $2000 - $2200
Duration: 32-week contract
Shift: 08:00 AM - 04:30 PM
Guaranteed Hours: 40 per week
- Assess plan implement coordinate monitor and evaluate options and services to meet patient healthcare needs.
- Develop and maintain individualized care plans in collaboration with patients families and healthcare providers.
- Coordinate with physicians nurses social workers and other healthcare professionals to ensure proper patient care.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Advocate for the patient and family ensuring access to resources and services.
- Ensure documentation is accurate timely and in compliance with hospital and regulatory guidelines.
- Support discharge planning and transitions of care including follow-up appointments and referrals.
- Experience:
- Minimum 1 year of experience as an RN Case Manager
- OR 3 years of experience as a Registered Nurse (RN)
- Licensure:
- Active RN license in the state of New Mexico or compact state license
- Certifications (Required):
- BLS (Basic Life Support)
- PALS (Pediatric Advanced Life Support)
- Certifications (Preferred but not required):
- ACLS (Advanced Cardiovascular Life Support)
- NRP (Neonatal Resuscitation Program)
- TNCC (Trauma Nursing Core Course)
- CNOR (Certified Nurse Operating Room)
