Job Description
Special Living Services Case Manager
The Special Living Services (SLS) program at The Arc Fond du Lac is a communitybased residential program for people with intellectual and developmental disabilities (IDD) age 18 and older. SLS supports people with IDD in living as independently as possible in their community with access to 24hour care and services.
The SLS Case Manager is responsible for managing aspects of The Arc Fond du Lacs Adult Family Homes and staff. SLS Case Managers are responsible for the care and supervision of intellectually and developmentally disabled (IDD) adults living in a communitybased residential setting. SLS Case Managers will assist in maintaining the homes compliance with regulations set forth by the Department of Health Services; to ensure licensing continuation and exceptional client care. Special Living Services is a 24hour 7 days per week operation. Hours may vary. All SLS employees are required to work some nights and weekends.
Qualifications
Required Knowledge Skills and Abilities
- High School Graduate. At least 18 years of age.
- Current WI drivers license and acceptable driving record.
- Working knowledge of computers computer programs typing and data entry.
- Ability to utilize Google Drive and Microsoft programs.
- Dependable detailoriented peopleoriented and teamoriented.
- Complete a background check and preemployment drug screening.
Preferred Knowledge Skills and Abilities
- Bachelors degree in Human Services Social Work or related field.
- Two years experience working with persons with disabilities.
- Experience with managing crisis situations.
- One years experience with staff supervision.
