Job Description

Overview

The MedServe Fellow works within the scope of essential functions at the Cone Health Center for Children caring for patients. Working under close supervision this position provides communityoriented primary care of the highest quality to all patients.

Responsibilities


Supports the physician in providing information about patients and preparation before patients come into the office.
Prepares key routine milestone preparation work for children including immunizations due ageappropriate screening tests and growth chart assessments.
Assists with immediate care coordination tasks generated during a patient care appointment such as previsit work diabetes check and information sharing and coordination with stakeholders in patient health.
Helps supervising physician create templates and processes for common tasks in the Care Coordination Center.
Supports the completion of care coordination communication activities including referral calls to specialists calls to school officials or teachers and other care team providers.
Performs other duties as assigned.

Qualifications


EDUCATION:Required: Bachelors degree
EXPERIENCE:Required: Graduate of an accredited Bachelors degree program
