Be a part of aworldclass academichealthcare systemUChicago Medicine as aClinical Pharmacy Specialistin Organ Transplantation. The Department of Pharmacy at the University of Chicago Medicine provides comprehensive pharmacy services to adult and pediatric patients.
Clinical Pharmacy Specialists in Organ Transplantation provide decentralized clinical services to adult and pediatric recipients of kidney pancreas liver heart and lung transplant as well as living organ donors in both the inpatient and outpatient settings. Core functions include completing pretransplant pharmacy evaluation providing posttransplant care as part of the multidisciplinary team in both the inpatient and clinic settings managing transitions of care providing medicationrelated education to patients and healthcare team members precepting pharmacy students and residents and maintaining programmatic infrastructure including protocol development outcomes review and quality assurance.
Job Summary
Through various assignments within the department Clinical Pharmacy Specialists support centralized and decentralized medicationuse systems and deliver optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacy Specialists are able to participate in all necessary aspects of the medicationuse system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs incorporating age and disease specific characteristics into drug therapy and patient education adjusting care according to patient response and providing clinical interventions to detect mitigate and prevent medication adverse events. Clinical Pharmacy Specialists serve as departmental resources and liaisons to other departments hospital personnel or external groups. They proficiently manage and facilitate the appropriate use of medications according to professional and regulatory standards. They conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medicationuse processes or pharmacy practice. Clinical Pharmacy Specialists provide medication and practicerelated education/training and actively serve as preceptors for Doctor of Pharmacy students and pharmacy residents.

In this salaried role we anticipate that you will generally work 40 hours per week Monday through Friday. We anticipate that you will work approximately six weekends and one holiday per year. We anticipate that you will be scheduled to work 100 at the Hyde Park campus.

Compensation & Benefits Overview
UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a fulltime equivalent 1.0 FTE) and is reflective of current market data reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
