Open Door Family Medical Center provides health care and wellness programs to individuals and families in need throughout Westchester Putnam and Ulster Counties. A pioneer Federally Qualified Health Center Open Door provides more than 300000 patient visits annually and serves nearly 60000 individual patients who might not otherwise have access. Founded as a free clinic in 1972 Open Doors mission of building healthier communities through accessible equitable culturally competent health care has led to site and service expansions. Open Door is currently seeking a Prevention Specialist in our Ossining medical center. The Site Medical Director has overall responsibility for providing and ensuring quality medical care to patients at medical site including the direct management and supervision of site medical clinicians. Recognizing the need to balance the needs of the Residents and the patients in need of care as an Open Door leader seeks to promote Open Doors patientcentered values and strategies.

DUTIES AND RESPONSIBILITIES

  • Identify and outreach potential candidates for comprehensive sexual health education (HIV/STIs/HCV) and PrEP/PEP navigation and support program (individuals at high risk for HIV infection) by using Open Door data reports collaboration with medical providers and outreach activities
  • Provide education to clients on prevention topics including HIV/STI/HCV/PrEP/PEP consistent and correct condom use safer sex practices harm reduction regular STI and HIV testing and the importance of mental health and substance use issues in relation to HIV risk
  • Refer individuals for HIV/STI/HCV testing and collaborate with medical providers to coordinate immediate followup after reactive/positive results including navigation to medical care (including other testing) and assistance with partner notification; for patients diagnosed with HIV conduct a warm handoff to ongoing case management or provide ongoing case management services
  • Coordinate clinical assessments and provide education/support for individuals interested in PrEP; for those who choose to engage in PrEP care provide education on selfmanagement the medication regimen and managing potential side effects treatment adherence support schedule followup visits send reminders and followup on noshows and assist with affording PrEP meds and visits by linking to insurance enrollment or assisting with PrEPAP and pharmaceutical assistance program paperwork
  • Coordinate care for patients in need of PEP services and discuss PrEP for future protection; navigate to PrEP care as patients are interested
  • Conduct Social Determinants of Health (SDOH) assessments to identify barriers to care/treatment and provide navigation and linkage to needed supports and services with an emphasis on physical sexual mental and behavioral healthcare/psychosocial counseling
  • Provide assistance with appointment scheduling accompaniment and appointment reminders as needed
  • Provide ongoing case management including assessments care planning navigation education and support; facilitates coordinates and documents participantcentered care for participants across Open Door teams and departments to better address multiple and cooccurring concerns
  • Conduct group presentations at communitybased organizations and health and human service agencies to increase knowledge of HIV/STI/HCV/PrEP and engage individuals into care services
  • Maintain electronic medical records in compliance with Open Doors standards in a timely manner
  • Meet program goals and targets as set forth by grantors and Open Door
  • Maintain program data evaluation metrics deliver reports and completed data entry accurately and in a timely manner
  • Participate in the development and implementation of performance improvement initiatives and ensure that such initiatives are supported with timely and accurate data collection and consistent actions
  • Other duties as assigned

QUALIFICATIONS

EDUCATION

  • Bachelors degree in an applicable field
  • Candidates with an Associates degree and significant applicable experience will be considered

  • Fluent in English and Spanish

  • Comfortable working in a fastpaced high accountability environment

EXPERIENCE

  • Experience with community/health/social service organizations preferred
  • Experience providing care coordination/case management and health education preferred
  • Experience with electronic medical records and data entry/tracking preferred
  • Knowledge and understanding of the issues needs and interests of LGBTQ community including knowledge of the spectrum of gender identity and transgender issues

REQUIREMENTS

  • Must have valid New York State driver license and daily access to car
