Qualifications:
- Registered Nurse (RN) license with a minimum of 3 years of healthcare experience preferably in hospice or related settings.
- Bachelors degree strongly preferred.
- Leadership or management experience a major plus.
- Valid drivers license (required)
Responsibilities:
- Lead and oversee an interdisciplinary team of nurses aides social workers chaplains and more.
- Ensure adherence to hospice regulations and standards.
- Facilitate communication between medical staff and the care team.
- Lead interdisciplinary meetings documenting patient needs and care plans.
- Develop and update patient care policies and emergency procedures.
- Manage staff scheduling performance reviews and foster a positive work environment.
- Act as the central hub for communication with families physicians and other providers.
- Analyze quality metrics and manage a caseload of patients as needed.
- Participate in afterhours and oncall duties to ensure seamless care.
- Maximize resource utilization and promote a collaborative work culture.
