Case Manager Opportunity
- Collaborate with a multidisciplinary team to create and implement personalized treatment and discharge plans
- Coordinate services that align with patient needs payer guidelines and clinical goals to ensure seamless care transitions
- Monitor and improve the patient experience by focusing on quality timeliness and continuity of care
Case Manager Qualifications:
- Must be a licensed Registered Nurse (RN) or Licensed Social Worker (LSW/LCSW)
