Our founders recognized a need for a nonprofit to step in to assist this deserving population and we are still doing that critical work today on Oahu Kauai and the Island of Hawaii. Role Overview

Join our team as a Transitional Case Manager for the Youth Kauhale Program where youll play a key role in supporting young adults experiencing homelessness on their journey to independence. Youll manage cases coordinate with a diverse team and ensure participants receive the services they need including housing medical care and employment resources. Your work will involve conducting assessments developing service plans advocating for clients in meetings and connecting them to vital community resources. This is a dynamic role where youll have the opportunity to make a meaningful impact on the lives of young people while working in a collaborative and supportive environment. Qualifications

  • A Bachelors Degree in Social Work Psychology or other related human service fields is required.
  • Experience with youth or young adults is highly preferred.

Requirement

  • A valid drivers license and access to a vehicle for transporting young adults.
  • Current auto insurance and safety check.
