We are seeking a compassionate and dedicated CommunityBased Support Direct Support Provider to join our team and play a vital role in supporting individuals with developmental disabilities!

Your Duties:

Provide direct care and support to individuals in their homes neighborhoods and during community outings fostering a sense of independence and inclusion.
Assist individuals with daily living activities such as meal preparation personal hygiene medication management (if trained and certified) and household chores ensuring their wellbeing.
Be a champion for community integration! Accompany individuals to social events recreational activities volunteer opportunities and educational classes expanding their social connections and life experiences.
Implement positive behavior support plans to promote positive behaviors and enhance individuals independence and participation in the community.
Maintain accurate documentation of daily activities progress notes incidents and observations keeping the team informed and ensuring continuity of care.
Collaborate effectively with support coordinators healthcare professionals family members and team members to provide holistic support and promote the wellbeing of individuals with developmental disabilities.

The Ideal Candidate:

Holds a high school diploma or equivalent; a degree or relevant certifications in human services psychology social work or a related field are a plus.
Speaks Spanish (Preferred).
Has previous experience working with individuals with developmental disabilities or in a similar caregiving role (preferred).
Possesses excellent communication interpersonal and collaboration skills.
Demonstrates a patient compassionate and positive attitude.
Is reliable dependable and highly motivated.
