What Youll Do in This Role:

The Concierge Provides nonclinical home care support to participants following interdisciplinary team guidance. Services are 1:1 and delivered in participants homes or contracted settings (Adult Foster Care Assisted Living or Skilled Nursing Facility).

Primary Responsibilities:

• Provide 1:1 supervision and companionship to participants.
• Facilitate activities and offer gentle redirection as needed.
• Perform light housekeeping (e.g. washing dishes sweeping tidying up).
• Prepare nutritious and satisfying meals.
• Accompany participants on bus rides to homes the PACE Center or appointments.
• Assist in the Day Center as needed.

What Youll Need:

• At least one year of experience working with frail elderly or longterm care populations.
• Reliable transportation and a valid Michigan Drivers License.
• Possess current CPR certification or willing and able to obtain as offered by LifeCircles.
