Due to successful internal progression opportunities we currently have a number of permanent fulltime vacancies forexperienced social workersacross our Adult Social Care community locality teams. We are looking for social workers who have completed the ASYE program and hold post qualifying are looking for enthusiastic and committed professionals to join our teams to help shape and deliver our services and contribute to the development of a model of best practice.
The successful applicant will be skilled in carrying out strengthsbased assessments and support planning under the Care Act 2014 completing comprehensive Risk Assessments Mental Capacity Assessments and Best Interest Decision Making. You should have experience of working within a multidisciplinary setting to include health partners voluntary and community organisations.

It is essential that as frontline practitioners in Adult Social Care you have time and space to reflect on and examine the work you do with adults families and carers to think through the work itself and your practice and the way this affects you.
