About the role
The Family Support Practitioner will work with families in partnership to empower and build their agency to make effective change and overcome their identified family challenges. The role is heavily outreach based engaging families in their homes and translating initial engagement into a strong ongoing partnership. Once engaged the role will not identify the family issues in a practitionerled assessment rather deliver interventions that build family awareness and insight into their history and current family challenges in order for families to selfidentify their key challenges and lead their own change.
