Job Description

Job Summary
Responsible for the coordination of patient care in a multidisciplinary clinical setting for comprehensive care of individuals with PraderWilli syndrome. Works with various providers and services to ensure the best possible treatment and outcome for patients. Coordinates necessary patient care services with physicians therapists lab radiology clinics and other specialists. This position is not responsible for providing patient care. This parttime position qualifies for the following benefits: U of U 401(a) employer retirement plan (immediate vesting) Employee supplemental retirement plan Health care plan (will pay parttime employee rate) Flexible spending accounts Life/Accidental Death and Dismemberment insurance Longterm disability insurance Group legal plan Holiday pay (will be prorated based on FTE Information on University of Utah benefits can be viewed at: Directs the scheduling of patient appointments with providers and ancillary services for a clinic every other month and as needed. 2. Follows up on patients treatment ensuring that patient needs are being met and that they understand the course of their treatment. 3. Triages patient questions and referrals and ensures appropriate coordination and follow through. Be available to the families to help coordinate appointments and provide needed care information. 4. Work closely with the parent support group the Utah Prader Willi Syndrome Association. 5. Gathers needed medical information to assist in the evaluation of patient needs. 6. Coordinates treatmentplanning conferences and presentations. 7. Assists with communicating the treatment plan to patients primary care physicians and needed specialists. 8. Acts as a liaison with other care groups such as social work schools therapists and nutrition. 9. Participates in discussions in forming guidelines in care management. 10. Collects baseline compliance measurements. 11. Participates in development and implementation of strategies to improve caregiver compliance. 12. Other duties as required. Problem Solving Challenges encountered by this position include complex scheduling with multiple physicians coordinating treatmentplanning conferences for physicians locating missing medical records and helping patients resolve billing issues. This position is expected to determine how to process the patient through the system in the most effective way. This may include deciding which provider will see the patient and which physician to contact regarding healthrelated problems patients may experience. Work Environment and Level of Frequency that may be required: Nearly Continuously: Office environment. Seldom: Infectious disease oils (there is air or skin exposure to oils or other cutting fluids). Physical Requirements and Level of Frequency that may be required Nearly Continuously: Hearing listening talking. Often: Repetitive hand motion (such as typing) walking standing sitting. Minimum Qualifications
Bachelors degree in Health Care Administration Business Communication Nursing or related field or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated human relation and effective communication skills are also required. Experience in oncology multidisciplinary clinics and/or working with highacuity complex patients may be preferred. This position is patientsensitive and must fulfill all associated requirements.
