Job Description

The FOCUS Implementation Specialist (Local Coach and Trainer) is part of a specialized team responsible for providing intermediate care management by working collaboratively with members and providers to ensure effective and holistic membercentered care. The FOCUS Coach will be provided training and supervision required to earn both their FOCUS Coach and Trainer certifications within 18 months.

As a member of CBHs Complex Care team the FOCUS Local Coach and Trainer will be responsible for monitoring and enhancing the quality of the FOCUS care coordination intervention provided by FOCUS case managers at provider agencies. By collecting and analyzing key data points monthly the FOCUS local Coach and Trainer will assess the strengths and needs of each FOCUS case management team and will tailor coaching sessions and the trainings offered at various intervals to meet the needs of case managers and their supervisors. The FOCUS Local Coach and Trainer will also support the Clinical Care Manager Specialists on the Childrens Complex Care team with monitoring monthly Plan of Care completion and utilization.


Essential Functions:

  • Provide processoriented coaching at least once per month to all local supervisor candidates as needed coaching will be conducted at the provider agency office or live in the field to assess a FOCUS case managers performance
  • Participate in monthly processoriented FOCUS coaching with national FOCUS coaches from UCONN Innovations Institute to perfect coaching skills
  • Develop proficiency utilizing all practice improvement tools (CREST FSI SAS CCRSR FACCS) to support skill building towards quality and fidelity FOCUS care coordination practices
  • Coscore at least one FSI and SAS per local FOCUS case manager per month
  • Cotrains and/or train (upon certification) all required FOCUS trainings and support the development of booster trainings/group supervision topics towards fidelity practice
  • Support onboarding of new FOCUS staff including CBH Clinical Care Manager Specialists and FOCUS supervisors and case managers at provider agencies
  • Collect monthly reports of key data points from FOCUS provider agencies analyze for practice trends and report out accordingly
  • Meet with CBH and provider agency leadership to inform about practice trends and brainstorm solutions
  • Submit documentation including recorded coaching sessions and all associated documentation and scored tools to UCONN national FOCUS coach at least once monthly in preparation for national coaching session
  • Participate in or lead FOCUS overviews for provider agencies and system partners as requested

Position Requirements:

  • Education: Masters degree in Social Work or related field
  • Clinical Licensure: Qualified candidates are required to obtain license within the first 18 months 1.5 years) of employment; PA Child Abuse Clearance required
  • Required Licensure: Licensed Social Worker (LSW) Licensed Professional Counselor (LPC) Licensed Marriage and Family Therapist (LMFT) Licensed Clinical Social Worker (LCSW)
  • Relevant Work Experience:
  • Minimum of 3 years of direct clinical care experience working with adults children adolescents and/or their families in behavioral health settings and experience in assessment and treatment planning; and/or experience in a managed care setting
  • Minimum of 1 year of experience training mentoring supervising or coaching other behavioral health clinicians

Work Conditions (including travel overtime required physical requirements and occupational exposure):

  • Must be able to regularly travel within Philadelphia and surrounding counties.
  • Regularly requires walking standing stooping bending sitting reaching pushing pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day.
  • Ability to utilize computers and office equipment to complete daily work responsibilities.
  • Regularly works inside in areas that are adequately lighted and ventilated. Some fluctuation in temperature. No protective equipment routinely needed.
  • Must be able to adapt to continuous changes/demands of the job.

Philadelphia Residency Requirement:

  • The successful candidate must be a current Philadelphia resident or become a resident within six months of hire.
