All Embracing Home Care is seeking a compassionate and organized Program Coordinator for Individuals with Developmental Disabilities to oversee and coordinate personcentered services for individuals with developmental disabilities. This role ensures that our individuals receive highquality individualized care that promotes independence and integration into the community. The Program Coordinator will work closely with individuals families service providers and regulatory agencies to develop and implement individualized service plans while ensuring compliance with North Dakota state requirements for a Developmental Disabilities Program Manager (DDPM).
Key Responsibilities:
- Service Coordination: Ensure individuals receive appropriate highquality care and services that align with their needs preferences and goals.
- PersonCentered Planning: Develop implement and monitor individualized service plans (ISPs) in collaboration with our individuals families and interdisciplinary teams.
- Regulatory Compliance: Ensure all services adhere to North Dakotas state and federal regulations governing developmental disability services.
- Team Leadership: Provide supervision training and guidance to Care Managers to promote best practices in service delivery.
- Advocacy & Support: Act as an advocate for individuals ensuring their rights and choices are respected while fostering selfdetermination.
- Budget & Resource Management: Effectively manage program resources and budgets to meet the individuals needs and maintain operational efficiency.
- Documentation & Reporting: Maintain accurate and confidential records including progress notes assessments incident reports and required state documentation.
Minimum Qualifications:
- Education: Bachelors degree in social work psychology counseling nursing occupational therapy or a related human services field.
- Licensure: Valid drivers license and the ability to meet agency driving requirements.
Preferred Qualifications:
- Knowledge: Familiarity with North Dakotas Developmental Disabilities Program Management (DDPM) requirements Medicaid waiver programs and personcentered planning.
- Experience: At least one year of direct care experience working with individuals with developmental disabilities or mental health conditions.
- Advanced Education: Masters degree in a related field.
- Certifications: Qualified Intellectual Disabilities Professional (QIDP) certification or equivalent.
- Additional Experience: Experience in program management staff supervision or working within home and communitybased services (HCBS).

Work Environment & Schedule:
- Setting: Officebased with regular visits to individuals homes community sites and service providers.
- Travel: Some travel required.
- Hours: Fulltime position; occasional evenings and weekends may be needed based on individual needs. Along with participation in the oncall weekend rotation.
