Description

About the role

The Youth Outreach Housing Specialist will work with Young People in partnership to empower and build their agency to make effective change and overcome their identified housing and wellbeing needs. The role is heavily outreach based engaging Young People in crisis accommodation street outreach and refuges and translating initial engagement into a strong ongoing partnership.

Duties

  • Meet individual case performance targets as per weekly monthly and quarterly dashboard system.
  • Provide high quality and intentbased practice in all interactions with clients consistent with the ATSICHS values and vision.
  • Provide high quality case management practices and record keeping that adhere to operational requirements.
  • Demonstrate positive and warm engagement through an assertive outreach approach that engages clients into the service.
  • Facilitate a clientled assessment and Care Plan to identify the clients goals including the entire family where appropriate.
  • Provide rolespecific targeted interventions and family facilitated group meetings in achieving client goals.

Skills And Experiences

COVID19 Requirement

  • Immunisation history showingyou have receivedtwo doses of a Covid 19 vaccine is encouraged.

  • Current Bachelors Degree in Social Work Human Services Social Science Health Allied Health Youth Work Community Services Family Intervention or Criminology with 4 years relevant experience in family support child protection youth support or related field.

OR

  • Current Diploma in Community Services Social Work Human Services Child and Family Allied Health Youth Work Child and Family Intervention with 5 years relevant experience in family support child protection youth support or related field.

AND

With at least 3 years of abovementioned experience in a case management role.

  • Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly monthly and quarterly performance targets.
  • Demonstrated ability to build client agency selfefficacy and facilitate change through client empowerment while actively discouraging client dependence on supports.
  • Demonstrated knowledge and ability to provide behaviour change interventions that build client selfawareness and active change in achieving their identified goals.
  • Demonstrated ability to work with the wholeoffamily irrespective of primary client in meeting the totality of family needs.
  • Demonstrated ability in coordinating multiple services in providing access to holistic care for clients particularly integrating internal agency supports across multiple teams.
  • Demonstrated knowledge and understanding of the factors contributing to vulnerability disengagement and intergenerational trauma of client group.
  • Experience with case management for high risk and/or crisis clients with highlevel planning and organisational skills.
  • Knowledge of the supports available to target clients in the areas of mental health substance misuse education employment and housing.

Additional Requirements

  • Requirement to travel across the Logan Bayside and Brisbane region as per program/resident location.
  • Current drivers licence.
  • Valid police check.
  • Flexibility to work outside core 8:30am5pm business hours in meeting individual client needs and provide responsive care.
  • Working with Childrens Card (Blue Card) or be eligible to apply.
