Description

In accordance with the Vision Purpose and Values and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Working in the community setting and as part of a multidisciplinary team the Geriatric Outreach Nurse acts as a special resource in psychogeriatric care including concurrent disorders; provides consultation and formal teaching; is responsible for the provision of clinical services to the psychogeriatric client through assessment treatment monitoring and directing care; act as liaison with community agencies and care providers; and assists in community development activities.

QUALIFICATIONS:

Education Training And Experience

Graduation from a recognized nursing program or evidence of an equivalent combination of education training and experience. Three 3 to five 5 years recent related experience including two 2 years relevant clinical experience. Registration with BC College of Nurses and Midwives as a practicing RN registrant or a practicing RPN registrant. Knowledge of current theories technologies and practices in the psychogeriatric care area; of applicable legislation such as the Mental Health Act and Adult Guardianship Act; and of adult education principles. Valid BC Drivers License.

  • Demonstrated contribution to the professional and the organizational vision as it pertains to psychogeriatric clinical practice and patient/client care;
  • Demonstrated critical and analytical thinking decision making and problem solving skills;
  • Demonstrated commitment to collaborative practice and life long learning;
  • Computer skills including the ability to use MS Office and other related computer technology;
  • A sound understanding of current and future nursing and health care issues and the potential impacts on nursing education and clinical practice;
  • Ability to operate related equipment; and
  • Physical ability to perform the duties of the position.
