STATE TEACHER II - School Social Worker Campus at Mount View Youth Services Center

• Support internal and external MultiTiered Systems of Support (MTSS) systems
• Monitor forms; assist others to create them
• Facilitate meetings
• Meet with EAS & proctor; review assessment reports
• Provide reminders to team
• Ensure student involvement
• Assist in communication w/BHS
• Spotcheck forms
• Document data on SmartSheets
• Keep up the ineligibility spreadsheet
• Weekly: new students
• Quarterly: site testing
• Share forms at BID meetings when applicable
• Social / emotional assessments
• Planning and collaboration with teachers regarding services and interventions
• Utilize the consultation model of services for teachers and administration
• Develop of systems and datatracking mechanisms for mental health progress monitoring and behavioral progress monitoring
• Design and implement schoolbased programs to promote a positive school climate in conjunction with the school administration team.
• Utilize social work training and skills to remedy barriers to learning by providing evidence based supports to school personnel
• Develop behavioral intervention strategies either student specific or schoolwide
• Lead and train others to facilitate restorative justice circles within the school environment
• Participate in conflict resolution and anger management as necessary in the school
• Participation in appropriate MDTs

• Facilitybased education staff Behavioral Mental Health Partners Assessment Specialists Client Managers
• Behavioral Health staff and Clinical Director to Consult with facility (including clinical & behavior mental health staff) and school staff regarding the implementation of researchbased interventions for students experiencing mental health and/or behavioral related concerns/issues as appropriate: which may involve modification recommendations of the classroom environment
• This may include supporting the creation of IGCPs for students during the school day
• Develop relationships with facility transition workers and assist with education transitions for youth
• Develop and implement behavioral plans to create cohesive support plans between the education and milieu environments.

• Provide school based mental health service provider minutes for students as outline on Individualized Education Programs
• Planning and implementing psychoeducational groups during the school day in both a preventative and remediation model to targeted students for whom behavior is impacting learning
• Prepare social developmental histories for initial or reevaluation meetings.
• Participate in Manifestation Determination meetings Crisis Response Team meetings
• Formally and informally assesses youth for special education evaluations
• Observe and lead the Functional Behavioral Analysis process
• Lead the Behavior Intervention Plan process both in writing and supporting
• 504 Site Coordinator obligations of the school.
• Participating in and helping with the regularly scheduled special education team meetings.
• Consults with other team members when determining necessary formal and informal behavioral and related service assessments.

• Classroom substitution using substitute lessons plans computerized curriculum or social/emotional curriculum.
• Maintain direct and ongoing communication with supervisor to bolster ongoing mental health support services and interventions within all DYS commitment facilities.
• Identify and report special education program/systemic strengths and limitations to supervisor.
• Participate in program development activities
• Train stakeholders on topics related to school social work services as well as assigned committees designed to ensure continuous quality improvement.
• Additional school based supports may include but not limited to interdisciplinary team problem solving intervention plan development school improvement planning prevention services teach or support affective education classes schoolrelated concerns such as academic achievement bullying harassment misbehavior or underachievement and family issues

• Bachelors degree from an accredited institution in a field of study related to the work assignment.
• Current valid Professional Teacher license issued by the Colorado Department of Education.
• Two 2 years of professional experience in an occupational field related to the work assigned to the position.

• Strong organizational and time management skills.
• Experience developing implementing and/or monitoring educational programming standardsbased curriculum and traumaresponsive/restorative environments.
• Emotional intelligence.
• Experience with atrisk youth

• Colorado Academic Standards for K12 education.
• Demonstrated knowledge in Microsoft Word PowerPoint and Excel.
• Attention to detail.
• Ability to build relationships with stakeholders.
• Strong decisionmaking skills and follow through.
• Ability to positively and professionally interact withadministrators staff parents clients community members and the judicial system.
• Excellentoral communication skills.
• Ability to collaboratively problem solve ask for and use feedback and create positive culture and climate.

• Must be 21 years old or older
• Must have no felony convictions and no violent history
• Must travel to training events and shadowing events in Denver area and other state facilities
• Must be able to pass Escape and Evade
• Must pass a Colorado Bureau of Investigations and a Federal Bureau of Investigations background check as a condition of employment.
