Job Description
Qualifications:
The successful applicant shall have or be eligible for certification in Family Medicine with the Canadian College of Family Physicians and hold active licensure with the College of Physicians and Surgeons of Alberta with acceptable experience in Family Medicine.
Description:
Alberta Health Services Department of Family Medicine is inviting applications from physicians interested in providing Family Medicine Care and
joinning our team at the Comprehensive Home Option of Integrated Care for the Elderly Dickinsfield (CHOICE CCD).
CHOICE CCD provides services for older adults who have complex health issues and want to remain living in their own homes. The goal of the program is to keep clients healthy at home and out of the hospital system.
CHOICE services may include:
- Medical monitoring and treatment by CHOICE physicians and nurses
- Medications dispensed by the CapitalCare pharmacy (via CHOICE)
- Physical and Occupational therapy
- Social Work
- Recreation (social activities)
- Personal Care (assistance with daily tasks as needed)
- Health and wellness education
