Position Summary:
Provides care management and care coordination for adult and pediatric patients with mild to complex illness in the primary care setting.
Reports to: Director
FLSA: Exempt
ESSENTIAL FUNCTIONS:
Identifies the targeted population and implements clinical interventions and protocols.
Collaborates with PCP patient and members of health care team including continuum of care settings and community resources.
Provides selfmanagement support and includes patient/family as active members in the team approach.
Facilitate close monitoring of high risk patients to prevent and/or intervene early during acute exacerbations.
Coordinates care between PCP and acute care hospitals specialists and post acute care services.
Minimum Qualifications:
Minimum Education
Associates degree in Nursing
Bachelors degree in Nursing (Preferred)
*Years of relevant experience may be substituted for required education.
Required Skills
Requires critical thinking skills decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Basic Life Support Health Care Provider (BLSHCP)
Licenses:
Registered Nurse (RN) in State of Michigan
