Responsibilities may include: - Coordinating with referral sources in managing and expediting all matters pertaining to incoming residential referrals
- Coordinating with employees to manage and expedite internal residential transitions and moves
- Establish and maintain effective team relationships with internal and external stakeholders
- Maintain complete accurate and accessible client and company files within the electronic records system
- Frequent interaction with external sources to foster business relationships
Requirements for this position include: - Bachelors degree in a social or human services field
- Three years ofin Indiana I/DD HCBS waiver system
- Experience in case management preferred
- Verification of good driving record valid drivers license current/unexpired auto insurance and reliable transportation
- Excellent written and verbal communication skills
- Proficiency of operating a computer and Microsoft Office and other required products.
- Willingness & ability to participate on a multidisciplined selfdirected team
- Empathy for people we support
- Ability to pass a Criminal Background
