Developmental Pathways (DP) has an opportunity for a Case Manager Active in our Case ManagementDepartment. This role will manage a caseload of individuals receiving Waiver and State General Fund services and maintain documentation needed quarterly and annually. The Case Manager Active will complete assessments to determine eligibility for services/supports and identify individual strengths needs concerns and preferences through interviewing observing and utilization of standardized tools. This role will also complete the required documentation related to monitoring activities to advocate for individual service needs and ensure the effectiveness of services delivered as authorized by the service plan. In addition this role will facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual needs. Lastly the Case Manager Active will provide case management services and support to temporarily assigned individuals and will support with department and team coverage as needed.
This role offers a 4day 40hour workweek with flexible scheduling options of either MondayThursday or TuesdayFriday. The role includes the flexibility to work remotely as well as the opportunity to collaborate inperson at our headquarters office in Aurora CO and/or at offsite locations.

Requirements
Bachelors Degree; or
Five 5 years of experience in the field of LongTerm Services and Supports (LTSS) which includes Developmental Disabilities; or
A combination of education and relevant experience appropriate to the requirements of the position
Valid Colorado motor vehicle license and provide proof of motor vehicle insurance
Meet our driving requirements no suspensions and no more than two 2 tickets in the past three 3 years
Upon request qualifying need and approval driving requirements may be waived in lieu of reliable transportation.
Preferred
One 1 year of experience in developmental disabilities/longterm services and supports
Additional Responsibilities
Schedule facilitate document and complete followup for all required meetings; disseminate updates and obstacles to the necessary team members
Make collateral contacts with such community resources such as law enforcement and medical personnel to obtain additional information coordinate services or secure their assistance
Ensure the accuracy and timely entry and distribution of required documentation to all necessary team members including but not limited to providers individuals and family members
