What Youll Do:
We are seeking a Population Health Analyst to be responsible for supporting the advancement of health equity initiatives through datadriven culturally informed programming that addresses the specific health needs of marginalized communities experiencing health disparities related to cardiovascular health. This role will focus on supporting programmatic development 70 and analytics 30 ensuring that all interventions are evidencebased and grounded in an understanding of social determinants of health.
We expect you to:
- Contribute to program development by employing culturally informed evidencebased interventions that address social service needs and advance health equity goals that address chronic disease conditions and other health needs of a diverse and underserved population.
- Provide technical assistance to community health worker navigators external partners and act as a subject matter expert in programmatic implementation to address chronic case needs among populations that are marginalized and disproportionately affected by chronic disease conditions.
- Work with stakeholders and community partners to prepare assessments analyze the data to identify key health issues in the community and share findings with relevant internal and external audiences.
- Organize and convene community partners and advocates to support equitable health outcomes fostering partnerships that promote education outreach and health equity in public health.
- Assist with engagement of the development of health equity curriculum health education promotion and outreach on a variety of public health topics that advance health equity.
- Prepare and deliver reports presentations and written communications with occasional travel (up to 25 for conferences meetings and relationshipbuilding activities.
Our requirements for this role:
- Bachelors degree required; Masters degree in public and/or population health health policy and/or health promotion or healthcare preferred. Equivalent years of experience and education may also be considered.
- Minimum of four 4 years of professional experience in health equity programming and analytics with expertise in public health and the social and economic factors affecting community health.
- Comprehensive knowledge of the healthcare delivery system and the role of communitybased organizations (CBOs) in advancing health equity.
- Proficiency in social and behavioral models community assessment tools program planning evaluation data collection and analysis to support stakeholder decisionmaking and promote equitable outcomes.
- Knowledge of measures and analytic approaches used to identify challenges and track improvements in equitable care.
- Knowledge of the challenges with the data and data collections systems for race ethnicity gender identity sexual orientation geography disability or preferred language.
- Requires superior written and verbal communication skills.
- Ability to incorporate guiding principles of quality improvement and health equity within scope of work.
- Demonstrated ability to establish and maintain collaborative and effective working relationships.
- High level of computer skills required including proficiency in Microsoft Suite (Word Excel PowerPoint Outlook etc. to include using social media platforms and website. This position will require significant computer use.
- Must be able to work well under pressure manage multiple tasks simultaneously work well in team situations meet deadlines and work independently.
