Primary Function of the Position

Hmong American Partnership is seeking to hire COVID Care Coordinators that want to support the safety health and wellbeing of the Hmong community. This position is primarily responsible for the delivery of the HAP COVID Community Coordinator Project within the Health and Wellness Department. The candidates must demonstrate a strong work ethic be customerservice driven be accountable and demonstrate excellence in quality work in previous work experiences. Primary responsibilities include: working closely with clients to help them access community resources enter information into enrollment tools and resources making appropriate referrals responding to questions about COVID testing outreach activities and staffing MNIT Amazon hotline. This position will provide inperson assistance and also work remotely to support the Hmong Community.

Position Responsibilities

  • Staff HAPs MNIT Amazon hotline daily during work hours and weekends and evenings as needed.
  • Answer calls and follow up on voice messages left on the MNIT Amazon hotline.
  • Attend orientation training for MNIT Amazon hotline.
  • Attend updated COVID training as needed by MDH and HAP.
  • Preregister members for testing on a regular basis.
  • Respond and followup to community questions about test sites registration questions/issues
  • Attend and be onsite to support provide coordination and translation support to local public health testing sites as needed.
  • Connect community members and clients to COVID resources and navigation services.
  • Provide information on COVID19 case investigation contact tracing and other information to clients and community members.
  • Navigate the COVID19 health system on behalf of the client and community members.
  • Support project coordinator in areas of outreach and community engagement e.g. drop off flyers and materials at distribution or testing sites
  • Enter client data into tracking sheet; ensure timely and accurate entries.
  • Participate and support coordinator in program logistics as needed e.g. gather materials or call partners to participate in testing sites.
  • Provide and fill out COVID19 application intake and referral process for MNsure.
  • Perform other duties as assigned.

Job Qualifications

  • Knowledge of the Hmong culture language and traditions required.
  • Desire to support clients and community members to stay safe and healthy.
  • Strong computer skills with experience in Office 365 and Microsoft Office e.g. Word Excel Power Point Share Point Teams.
  • High level of attention to details.
  • Excellent skills in written and verbal communication when working with clients and community members.
  • Ability to work with ambiguity and uncertainty.
  • Excellent customer service and conflict resolution skills.
  • Effective organizational skills.
  • Ability to prioritize urgent and immediate tasks appropriately.
  • Knowledge of data privacy and confidentiality standards and protocols.
  • Demonstrated ability to work and communicate effectively with people of diverse ethnic economic and racial and cultural backgrounds.
  • Team player and adaptable as needed for agency needs.
  • Position requires flexible scheduling remote work combined with going into the office on certain days including occasional weekends late evening events and travel within the 7County metropolitan area surrounding Minneapolis and St. Paul.
  • Must have a reliable car a valid drivers license automobile insurance and the willingness to use personal vehicle for work related travel.

Education and Experience

  • High School education or equivalent;
  • Years of experience: At least 12 years working in patient care health care or similar field
  • Computer skills necessary to use internet word processing spreadsheets and data base programs

  • While performing the duties of this job the employee is regularly required to stand sit; talk hear and use hands and fingers to operate a computer phone and keyboard
  • Must be able to remain in a stationary position for sustained periods of time in an indoor setting
  • Ability to tolerate moderate noise levels (i.e. shared open office space with computers phone and printers light traffic).
  • Must have the ability to move about the office to access files and office equipment
  • Frequent moving of boxes and files weighing up to 20 pounds
  • Requires occasional bending kneeling balancing and reaching
  • Able to focus on data analyze the information and draw conclusions
  • Frequent interactions with coworkers and participants requiring ability to communicate effectively
  • Frequent communication in writing and via email requiring ability to fluently read and write in English
  • Occasional preparation and analysis of figures
