Direct Support Professional Adult Service
Community Living Coach
Must have evening and weekend availability

But some require more assistance to find that type of life than others. Adults with intellectual disabilities may require coaches to help them develop the life they want. They may need someone who can show them what opportunities are out there. They definitely need someone they can trust. Someone that they can depend on.

But it aint easy. All of this comes with incredible responsibility. They may need assistance with things that other folks take for granted like bathing. They may require a lot of detective work to determine what is bothering them communication may not come easy. Sometimes they may not be in the best mood. Who is all the time
You will not do this alone. Grafton has been providing residential opportunities to adults for well over forty years. Our teams know what they are doing. The leadership team for adult services is second to NONE.

If you think that people with intellectual disabilities simply need firm guidance a consistent disciplinarian or not much stimulation or activity please do NOT apply. If you recognize that each and every one of us has experienced our own trauma are a bit of a puzzle to be solved as a result and have an innate right to compassion and kindness climb aboard.
Also a word of warning. Once you work in Adult services at Grafton you generally have a hard time working in any other job. Because its not really a job. Its a mission. Once you become a part of the life of one of our residents you are going to have a very hard time leaving here.
GENERAL STATEMENT OF JOB
TheDirect Support ProfessionalAdultprogramparticipatesin the direct care and plan implementation for clientsas prescribed byphysician and treatment teams. They provide supervisionsafetyand crisis management for the client population; complete documentation of client behavior and participation in programming; adhere to the scheduled activities; role model positive mature behavior;utilizeproactive behavioral strategies to minimize client crisis and maximize success.
ESSENTIAL JOB FUNCTIONS
- Ensures the healthsafetyandwellness of Clients
- Supports client health and hygieneandutilizesinfection control procedures.
- Reports serious incidentsparticipatesinreviewsand implements followup measures as applicable.
- Follows and administers the individual plan for clients to include understanding ofSNAP (strengths needsabilitiesand preferences) goals/objectives/outcomes and treatment strategies from the current Plans (Treatment Plan IEP ISP) of assigned clients.
- Collaborates with the Treatment Team to provide continuity of support and care by communicating and contributing to team discussions and decisions
- Promotes clients achievement of behavioral goals by implementingthe behaviorstrategies and teaching alternative behaviors asindicatedin the Treatment Plan/IEP/ISP.
- Actively instructs assigned clients so they can achieve education/residential goals andobjectivesor ID Waiver outcomes.
- Collects and documents behavioral and instructional data accurately and reliably.Documents servicesdeliveredaccurately andin a timely manner.
- Maintainsa high levelof engagement with and monitoring of clients. Implements planned activities throughout theworkday.
- Provides asupportive andsolutionFocused Environmentthrough building trust respect and good relationships with clients
- Manages behavioral crisis safely.
- Oversees site management to include housekeeping nutrition andtransportation.
- Assistswith purchases and is accountable for program and client money as applicable
MINIMUM REQUIREMENTS
- 18 years of age
- High school diploma or GED
- Valid drivers license with an acceptable driving record.
- Demonstrates moderate computer skills how to login how to navigate the web for information how to start and save document in Microsoft and has basic keyboarding skills.
COMPENSATION
- Starting at $15.50 up to 18.55/hour based on years of experience.
- Depending on hoursworked you may be eligible for Graftons shift differential that is currently being offered.
- Shift Differentials: $1.50/hr weekday evenings & overnight 3:30p 10:30p & $3.00/hr weekend Fri 3:30p Mon 7:00a
