We are looking for a bilingual LVN (Spanish) for one of our managed care clients here in the South Bay. They are looking for the following:
- Under Pharmacist supervision enhance seniors and their caregivers abilities to manage their medications more safely in the community to prevent unnecessary medicationrelated problems and negative health outcomes
You will
- Support the Pharmacist during medication management services by providing homebased and telephonic coaching on specific medication safety issues.
- Facilitate communication between the clients their caregivers and their providers/service providers to address gaps in safe medication management.
- Participate in departmental QA process and assist with program documentation.
- Under Pharmacist supervision maintain clients health by helping the client and caregiver understand and use medication and other healthrelated community services and reinforcing use of selfmanagement and medicationrelated organizational tools.
Under Pharmacist supervision support clients wellbeing by providing ongoing facetoface or telephonic visits with clients to identify barriers to safe medication use and supporting the plan of care developed.
Contribute to the management and development of an individualized care plan for the client by conducting home visits reporting and documenting homebased observations and providing approved coaching on identified medication safety issues as directed by the Pharmacist.
Under Pharmacist supervision assist clients and caregivers in taking an active role in safe medication management by coaching education navigation and referrals to pharmacy and community medication resources.
Identify pertinent information to communicate to care team by contacting client as needed.
Exhibit strong interpersonal critical thinking and analytical skills through positive communication with clients caregivers care team and community resource agencies/organizations including pharmacies.
Demonstrate excellent organizational decisionmaking and multitasking skills as demonstrated by problem solving and successful outcomes.
Complete timely and accurate documentation in multiple computer systems to record assessment observations and corresponding documentation including care plans and progress notes.- Enhance skills and knowledge by participating in educational offerings and team case conferences and training per department guidelines.
Your qualifications
- G.E.D. or High School Diploma
- Licensed Vocational Nurse required with current and active California license in good standing
- 2 years of in community services preferably a healthcare or home health environment.
- 2 years working with older individuals and their families.
- Working knowledge of medical and medication terminology and abbreviations
- Broad understanding of local community medicationrelated resources for seniors and caregivers
- Outstanding verbal and written communication skills
- Ability to work in a fastpaced every changing work environment
- Proficient in MS Office required (e.g. MS Word MS Excel)
- Strong analytical and critical thinking skills.
Your preferred qualifications
- Communitybased and homebased experience.
- Proficiency in a second language.
