Knowledge of Microsoft Word Excel and Power point required; certificate of completion of Microsoft Word Excel and Powepoint training preferred.
Must be comfortable working with various computer programs.
Minimum of two years of administrative support experience in a hospital is required.
Familiarity with medical terminology knowledge required. Excellent customer service skills critical. Effective interpersonal skills essential. Good telephone etiquette.
Associates or Bachelor of Science degree or equivalent experience preferred.
Previous work in healthcare environment preferred. Hospital or medical based administrative experience helpful. Experience working in multidisciplinary professional environment.
The position provides administrative support to the Manager and staff within the Care Coordination department as it relates to overall department scope of responsibility.
The Collaborative Care Coordination Assistant is responsible to work with a multidisciplinary team to optimize the discharge process and help to support a safe transition of care that is primarily directed by RN Case Managers and Social Workers. Additionally the CC assistant will act as a liaison to skilled nursing facilities and will communicate with external facilities via the electronic discharge software program i.e.4 Next in writing and telephonically as directed by the Manager RN Case managers and Social workers. The assistant will prepare discharge packets arrange ambulance transport help with facility/home care referrals arrange for DME and routinely finalize communication in 4 Next.
The job may involve going in patient care rooms.
PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:
Assist with the coordination and referral process for discharge planning as requested by the
Care Coordination Manager Case Managers and Social Workers.
One Weekend day (Saturday) per week is part of the work week.
