Were hiring for a Clinical Care Specialist for the First Episode Psychosis (FEP) Program to join us at 1012 W. Abriendo Ave Pueblo CO!

What Youd Be Doing

As a Case Manager you will work as part of a transdisciplinary team to provide supportive communitybased services to youth and young adults ages 1529 who meet criteria for the program. In addition to providing services and support to youth and their families the Clinical Care Specialist Case Manager will provide training education and support to the transdisciplinary team and community members as well as cofacilitate multifamily groups and other groups as appropriate

Under the supervision of the FEP Team Lead/Clinician the Clinical Care Specialist responsibilities are:

  • Outreach and engagement screening assessment diagnosis and strengthsbased treatment.
  • Planning psychoeducation and support for individuals family/primary support systems crisis support and relapse prevention planning independent living skills development benefits planning support for educational and vocational settings medication support and opportunities for selfadvocacy mentorship and socialization.
  • Deliver services that address diverse social cultural gender and economic needs of population served.
  • Participates in quarterly education workshops as well as community education and outreach as assigned.
  • Utilizes a recoveryoriented treatment modality for people experiencing their first episode of psychosis (FEP).
  • Identifies problem areas service gaps and areas that are barriers to services for individuals.
  • Develops initial psychosocial assessments diagnosis formulation crisis plans and treatment plans;
  • Knowledgeable about behavioral health programs evidencedbased interventions and the unique needs of transition age youth young adults and their families.
  • Experience working with diverse teams to identify policy needs plan analyze and solve complex problems as well as have a passion for early intervention and expertise implementing behavioral health programming in community setting for transition age youth young adults and their families;
  • The case manager adheres to program practice guidelines training and certification
  • Develops and maintains a professional working relationship with other community agencies;
  • Staff documents in a timely manner according to agency policies and procedures.
  • Maintains competency and proficiency with our electronic health record system;
  • Willing to work flexible hours in order to meet the needs of the clients and families;
  • Provides support to remain in home and community to avoid more restrictive living arrangement and
  • Performs other jobrelated duties as assigned.

Qualifications
