Job Description

Responsibilities include the coordination of care management providing information to vulnerable populations in various settings throughout the community assessing the emergency needs of each individual requesting service providing immediate direct services and working as part of the team for effective care management. Provide highly individualized services in the community and promote client selfdetermination and decisionmaking. Provide essential expertise and consultation to the team to promote a culture in which each individuals preferences are recognized and integrated into treatment rehabilitation and community selfhelp activities.

Scope of Duties:

  • Interact (verbally and nonverbally) with all clients staff and community members professionally with respect and courtesy.
  • Maintain client and staff confidentiality in compliance with administrative policies and procedures.
  • Attend and participate in regularly scheduled staff meetings supervisory conferences and training sessions and meetings as required.
  • Participate in regular street outreach activities and community events to engage individuals within the community.
  • Disseminate information regarding services provided by the agency to the community.
  • Responsible for the coordination of transportation for individuals requesting services.
  • Coordinate comprehensive intake assessment procedures on all engagements.
  • Communicates special needs and represents clients interests to program staff.
  • Network with other community agencies to develop resources for clients.
  • Complete Service Plans on all assigned clients based on approved agency assessment tools with client input.
  • Monitor and document client progress according to funding source standards and/or operational requirements in realtime.
  • Fulfill obligations of training agreements entered into for core curriculum programs.
  • Develop appropriate discharge plans for all assigned clients as per contractual requirements and policy and procedures.
  • Complete necessary reports and caserelated paperwork in a timely fashion.
  • Facilitate regular group sessions for clients as directed.
  • Provide practical assistance for clients when they first enter the facility by mentoring advocacy coordination sidebyside individualized support problemsolving and direct assistance to help clients obtain the necessities of daily living including utilizing nutritional financial physical mental and emotional resources.
  • Assist in maintaining communication between all staff members and clients.
  • Complete other duties as assigned.

Credentials/Abilities:
Must have a High school diploma or equivalent with a minimum of two yearss work experience in a relevant field. Must have direct experience in working with the homeless population and individuals who suffer from substance abuse and mental health issues. Must possess effective written and verbal communication skills and an ability to interact appropriately with clients. Must be able to pass a criminal background check. A valid drivers license is required and the ability to meet agency driving standards. Bilingual is a plus.
