Position Summary
The SUD Coordinator provides case management and care coordination within the hospital connecting individuals with substance use disorders to treatment and support services. This role ensures a seamless transition in care collaborating with hospital medical teams and linking patients to community resources for longterm recovery. Also assists with review and analysis of utilization data and monitoring of hospital procedures patient outcomes and other duties as defined by supervisor.
Essential Duties and Responsibilities
Is part of an SUD Response Team to include 24/7 coverage of the Emergency Services Department and additional areas as needed.
Completes screening evaluation and placement of patients.
Develop placement and discharge plans.
Coordinates the delivery of a broad range of services targeted at minimizing hospital utilization and maximizing treatment engagement.
Collaborates with hospital staff to improve the management of patients.
Coordinates Substance Use Disorder services provided to the Emergency Services Department and additional areas of the hospital as needed.
Participates in planning and successfully implements strategies to achieve the appropriate length of patient stay in acute care settings.
Plans and implements strategies and resources to achieve increased engagement of acutely intoxicated individuals in indicated treatment services.
Maintains consistent communication with hospitals medical team to update them on patient planning.
Identifies and coordinates the care planning for high utilizers of acute services and develops effective interventions to change default presentation from Emergency Services to identified communitybased services.
Interviews patients reviews records and confers with other professionals to evaluate the condition of patient.
Consult patients to assist patients in overcoming alcohol and drug dependency.
Consults family members to assist family in dealing with and providing support for patients.
Consults family members regarding the impact of alcohol and drug dependency in their lives.
Consults patients/family members in crisis as needed.
Refers patient to appropriate services as needed such as detoxification medical evaluation and treatment social services and recovery resources.
Maintains contact on a regular basis with referral sources.
Maintains patient files utilizing identified technical resources and providing timely documentation in the EMR in a site visit ready manner at all times.
Develops discharge/transition plans for patients going to SUD treatment.
Understands and implements all TAP 21 Addiction Counseling Competencies.
Participates in utilization review activities as required.
Reviews and sees patients in follow up folder and completes all steps toward treatment placement possible.

Education and Experience
Bachelors degree (B.A. B.S. or equivalent preferred; and four to ten years related experience and/or training; or equivalent combination of education certification and experience.
Language Skills
Ability to read analyze and interpret general business periodicals professional journals technical procedures or governmental regulations. Ability to write reports business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of managers patients customers and the general public.
Mathematical Skills
Ability to calculate figures and amounts such as discounts interest commissions proportions percentages area circumference and volume. Ability to apply concepts of basic algebra and geometry.

Certificates Licenses and Registrations
Current valid Kansas Drivers License.
Licensed by the State of Kansas Behavioral Science Regulatory Board as a Licensed Addictions Counselor (LAC) is preferred but not required.
