An exciting job role has recently become available in Bristol. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be
In this role you will be responsible for completing Care Act assessments mental capacity assessments and other assessments in line with legislation. You will develop comprehensive discharge plans and collaborate with medical staff to ensure seamless patient care transitions. Your work will be crucial in supporting patients needs and ensuring they receive the appropriate care and support during their transition from hospital to home or other care settings.

To be successful in this role you must have Social Work England registration be eligible to work in the UK hold a full UK driving licence possess postqualified experience and have a degree level or equivalent qualification in Social Work. These qualifications and experiences will ensure you are wellprepared to meet the demands of the position and provide highquality support to those in need.
