We are looking for a LeadQIDP Case Manager who demonstrates this mission and wants to work for an organization that makes a difference in the community.

What will you be doing
The Case Manager QIDPLead is responsible for the clinical case management of assigned caseload. Ensures that all aspects of community living comply with all applicable Federal State and Accreditation standards. Responsible for developing and implementing individual programs to maximize the clients independent community living skills and inclusion in their community. Provides documentation on assigned clients in accordance with DHS CARF or DPH standards as applicable. Advocate for and maintain rights and privacy of clients that are served. Accountable for providing to both clients and staff members a positive professional and safe environment to live learn and work.Responsible for a permanent caseload of up to 25 and additional cases as assigned based on the needs of the program. Supervise QIDPs and support staff as assigned. Provide staff training for client strategies and/or programs and assist with new hire training as assigned. Perform client case record reviews and advise QIDP on improvements as needed. Prepare a monthly accountability report as assigned. The Case Manager QIDP must be a role model for clients and staff for both permanent and temporary assignments including providing and participating in direct service as needed. This job position may have some work components that can be performed remotely. Remote work arrangements are not a right or entitlement of employment. They are discretionary and subject to demonstrated performance and operational needs. Approval may be rescinded at any time at the sole discretion of management. May work remotely for up to 20 of the work week with approval from the Clinical Manager.

Education:
Bachelors Degree in Human Services.
License/certifications:
Previous QIDP training highly preferred.
Experience:
- At least one year of experience working directly with individuals with developmental disabilities.
- Three to five years of experience as QIDP or QMHP preferred.
- One year of supervisory experience preferred.
Physical
- Climb stairs daily
- No medical restrictions which would prohibit implementation of a lift/transfer or implementation of behavioral programs.
- Push pull reach stoop/bend over and use step stool daily
- Perform daily housekeeping tasks and assist clients in independent living skills.
- Physically assist clients in situations including but not limited to: getting in and out of van bathtub shower or bed regularly
- Implement behavior support strategies and respond to emergency behavioral issues.
- Requires significant walking and standing on a daily basis.
Equipment:
- Computer and software including Microsoft Word Outlook Excel and Teams; Zoom platform and application; Internet browser and Microsoft SharePoint platform.
- Household appliances including stove microwave washer/dryer dishwasher and countertop appliances such as microwaves.
- Alarm Systems for monitoring fire weather security etc.
- Telephone and voicemail.
- Office Equipment including printer/scanner copy machine calculator shredder etc.
- Adaptive equipment as defined in individual strategies including hoyer lift and accessories shower chairs or benches walkers wheelchairs etc.
- Agency vehicles with and without wheelchair lifts.
Additional Requirements:
- Minimum age requirement: 21 years
- Must be able to lift or move up to 50 pounds to assist in twoperson lift or with use of adaptive equipment.
- Pass STOFHLA literacy test.
- Must complete required DSP and Medication Administration Training including CPR First Aid Infection Control OIG Rule 50 and CPI.
- Must maintain medication administration authorization by following all policies and procedures.
- Individual must have and maintain a valid Illinois drivers license.
- The use of a personal automobile and the minimum amount of automobile liability insurance as defined in the Associations personnel policy is required.
- Must transport clients in personal automobile when necessary.
- Must have access to a personal cell phone with ability to utilize downloaded applications for secure messaging.
- Complete agency required QIDP training within six months of hire.
- Respond to client medical/behavioral as well as facility concerns during evening weekend overnight and holiday hours.
- Evenings weekends and holiday hours required per client and program needs.
- Maintain 12 CEUs yearly.
- Attend inservice trainings as required.
- Attendance at afterhours programmatic functions required.
