Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing healthcare HIV prevention and treatment among other lifesaving services.

This position is included in a bargaining unit of Housing Works New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (RWDSU). Accordingly the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a CBA) between Housing Works and the RWDSU
Compensation Range $25.00/hr $31.25/hr commensurate with experience
Overview:
The Outreach Coordinator structures manages and supports the work done by the outreach workers and the assigned peer workers across harm reduction programs. This staff will ensure that the data and services provided are documented in a timely basis staff are properly trained on service interventions updates protocols as needed and makes sure participants are provided services as outlined in the scope of service for each program. This staff will assess and implement quality improvement processes to the work done by outreach staff. Structures required training for staff on an ongoing basis or as needed to build proficiency and professionalism. Builds a team with cohesive service goals and professional delivery of services.
The Outreach Coordinator makes sure that PDSE targeted case findings community level interventions and program promotion activities and conducted consistently and appropriately. This position will submit data as required by NY City and State funders including and not limited to AIRS EiCare EShare and Red Cap. The Outreach Coordinator will conduct services five days a week. Outreach services will include program promotion activities referrals prevention training provide education for safe injection practices and highrisk prevention services. Builds relationships and locates resources as well as promote steps to independence health and wellbeing of service users. Duties also include recruiting clients administering questionnaires policies and procedures updates data gathering assisting with event organizing and performing various administrative tasks including periodic reporting.
Responsibilities:
Primary:
- Conducts outreach routes and activities in assigned programs catchment areas
- Assess monthly team needs of program supplies and inventory.
- Organizes participation of outreach team members on community events as needed.
- Provides training and oversight to stipend workers interns and volunteers.
- Provides sterile injection equipment and safer use materials/information and Harm Reduction strategies available to PWU/ID.
- Assures that eligibility is met for each new participating for all program services.
- Delivers health promotion health education and risk reduction counseling interventions as needed.
- Is fully acquainted with all harm reduction programs services and enrollment requirements.
- Maintains routine communication and assists with coordinating the clients care navigation needs.
- Participate in case conference and clinical assessment meetings as needed.
- Completes and submits documentation of services provided in a timely and efficient manner (e.g. within two business days past the provision of service).
- Completes and submits weekly and monthly reports on service activities.
- Provide feedback on services completed barriers and community developments locations and types of service conducted.
- Develops & maintains an outreach log to track services conducted areas where outreach happened and demographics of individuals engaged.
- Adheres to workplace policies and procedures including confidentiality documentation channels of communication workplace culture dress code and conflict resolution procedures.
- Updates outreach policies and procedures document at least once per year or as needed.
- Must be knowledgeable about community resources including educational social and emotional support services available to clients.
Secondary:
- Attend meetings as deemed appropriate and related to the scope of service.
- Participates in quality assurance and quality improvement planning project and activities as directed.
- Able to present analytical program data to harm reduction staff and other stakeholders.
- Participates in monthly staff and peer worker development meetings and interventions as needed.
- High school diploma or General Equivalency Diploma required or higher and a minimum of five years of field work in this area or harm reduction.
- Bachelors or masters degree in human services preferably.
- Ability to work independently and oversee staff in outreach tasks.
- Make data entries into programs database such as AIRS eShare EiCare
- Good verbal written communication and interpersonal skills.
- Use of computer software Microsoft Office Suite including Word and Outlook calendaring and Excel.
- Culturally aware and sensitive to PWUD (People Who Use PLWHA (People Living with HIV/AIDS) LGBTQAI YIDU (Young Intravenous Drug Users) SMI (Severe Mental Illness) and MSM (Men who have Sex with Men)
- Preferable training in the following areas:
- Motivational Interviewing
- Syringe Exchange and Harm Reduction
- Needlestick prevention
- Outreach Safety
- HIV confidentiality
- Riskreduction counseling
- Deescalation Training
- Community Sensitivity
- Ability to make introductions and connections with members of the community.
- Safe injection training experience.
- Good verbal written communication and interpersonal skills.
- Use of computer software Microsoft Office Suite including Word and Outlook calendaring and Excel.
- Able to document services and information appropriately.
- Bilingual in Spanish/English preferable.
- Frequently walking to conduct services often standing for prolonged periods
- Ability to work outside in varying weather conditions and hours.
- Occasionally moving around to additional worksites.
- Often carrying lifting and moving objects up to 25 pounds.
