The Family Case Planner will carry out a variety of clinically driven preventive service casework activities in collaboration with the FTR Clinical Team. Coordination of services across the substance use mental and medical health and Family Court systems will also be required. Provide supportive services to children and families and written documentation as required.
About the Program
The Family Treatment Rehabilitation Program (FTR) located in Brooklyn NY offers intensive preventive services to families at risk of removal of children to foster care due to neglect and abuse. Risk factors for families served by the FTR program include mental health and substance use. Using Solution Based Casework we actively work with families to develop plans that make them stronger and keep them together and to address the Mental health and substance use issues impacting the family. We encourage families to build connections to positive supports in their community who will help them sustain their progress and can be in place after our services discontinue. Our goal is to partner with families to keep them together while ensuring that children will be safely cared for. We utilize a collaborative team approach to provide services to families and ensure families progress through the three Treatment phases of FTR.
Job Responsibilities
Welcoming clients to the office providing a trauma informed atmosphere for clients ensuring clients are escorted to and from sessions in building as needed.
Maintain a caseload of 8 family cases and address the mental health and substance use issues that they face. Conduct face to face contacts in the family homes and program site and maintain contact requirements as per Treatment phase.
Provide direct service to clients including but not limited to: Counseling around healthy coping skills/ harm reduction/impact of substances on parenting and psychoeducation.
Complete all other program required paperwork i.e. Progress Notes Program Family Assessments Family Assessment Service Plans (FASPs) and other reporting requirements as related to the FTR program model. Maintains timely documentation of assessments/progress notes/collateral outreach/ psychosocial/screenings via statewide and agency systems. PROMIS/Connections and OneDrive for ADVPO cases. All documentation completed by 10 days after event date. PROMIS is updated weekly. Maintains a calendar via Outlook.
Participate in weekly Supervision with a planned agenda on family case issues to discuss and follow through on supervisory directives. Be trained and implement a Solution Based Casework model approach.
Complete mandated trainings as per guidelines. Remain up to date on professional development trainings with a focus on mental health and substance use.
Monitor and document child contacts in a family for risk and safety as described by the Administration for Children Services (ACS) standards. Including but not limited to: Monitor children in their school placement and educational performance their medical needs. Assess refer and monitor children and families to mental health and/or substance abuse services.
Collaborate with program Case Aide in aiding and supporting families in obtaining resources support services and advocacy assistance and Clinician and/or CASAC in assessment of family members. Follow recommendations of assessments.
Attend and participate in any and all meetings and conference pertaining to family cases as assigned with other agencies child welfare and/or family court. Ensure families and children attend and participate in Family Team Conferences to promote case progress reduce length of services and improved outcomes for children
Participate in monthly Clinical Diagnostic Team (CDT) meetings to discuss family case progress and/or develop intervention strategies to improve family outcomes. Participate as a member of an interdisciplinary treatment team to facilitate the client recovery and rehabilitation processes.
Implement Quality Assurance data to drive case practice and improve performance and achieve outcomes
Attend ACS and agency inhouse trainings weekly supervision CDT FTCs staff meetings as directed.
Provide clinical support to Program Director/Program Supervisor/Clinical Coordinator to ensure that program is in compliance with OCFS and NYC Childrens Services policies and protocols.
Refer provide and assist with targeted interventions as needed for families and contribute to the healthy integrity of the programs therapeutic environment.
Coordinates treatment with other communitybased organizations as indicated.
Performs other duties as requested or assigned
Qualifications
Bachelors degree required with some child welfare experience.
Must be willing to do home visits.
Good verbal and written communication skills.
Bilingual (English/Spanish) preferred
